Soliter beyin metastazı ve glioblastoma multiforme arasında konvansiyonel MRG ile ayırıcı tanı yapmak çoğu zaman tanısal sorunlar ortaya çıkarır. Bu konuda difüzyon ağırlıklı görüntüleme ile ilgili çalışmalar olmakla birlikte sonuçlar değişken ve tutarsızdır. Bu retrospektif çalışmanın amacı, peritümöral minimum ADC değerleri ve ADC gradientlerinin glioblastoma multiforme ve soliter beyin metastazını ayırt etmedeki rolünü değerlendirmektir. On iki glioblastoma multiforme ve 31 soliter beyin metastazı tanılı toplamda 43 hasta cerrahi rezeksiyon öncesinde difüzyon ağırlıklı görüntüleme ile değerlendirildi. Peritümöral ödem içerisinden, tümöre en yakın, orta mesafede ve en uzak lokasyonlardan 3 adet ADC değeri ölçümü yapıldı ve bu değerler birbirinden çıkarılarak ADC gradientleri hesaplandı. Ayrıca peritümöral ve tümöral minimum ADC değerleri ile ADC oranları, ipsilateral ve kontralateral normal beyaz cevher ADC değerleri, BOS ADC değerleri her lezyon için kaydedildi. Peritümöral ödem dokusundan hesaplanan ADC gradienti değerlerinde glioblastoma multiforme ve soliter beyin metastazı arasında istatistiksel olarak anlamlı fark bulundu. Minimum peritümöral ve tümöral ADC değerleri ile peritümöral ve tümöral ADC oranlarında iki grup arasında istatiksel olarak anlamlı fark bulunmadı. Sonuç olarak difüzyon ağırlıklı görüntülemede peritümöral ADC gradienti ölçümü glioblastoma multiforme ve soliter beyin metastazlarını ayırmada güçlü ve kullanışlı bir yöntem olabilir.
Differentiation between glioblastoma multiforme and solitary metastatic lesions, two of the most common malignant brain neoplasms, is often a diagnostic dilemma with conventional MRI. The use of diffusion weighted imaging to better characterize peritumoral edema has been invastigated for this matter but the results has been variable and inconsistent. The aim of this retrospective study was the evaluate the potantial role of peritumoral minimum ADC values and ADC gradient values for the differentiating glioblastoma multiforme and solitary metastatic lesions of the brain. Forty three patients, 12 with glioblastoma multiforme and 31 with solitary brain metastasis underwent diffusion weighted MR imaging before surgical resection. The ADC values were calculated in peritumoral edema in three locations: near, an intermediate distance from and far from the enhancig mass. ADC gradients were calculated as the substractions of this three values. Minimum ADC values of tumoral lesions and peritumoral edema, ADC values of normal appearing contralateral and ipsilateral white matter and ADC values of CSF were also recorded for each lesion. The evaluation of the ADC gradient values revealed a statistically significant difference between glioblastoma multiforme and solitary metastatic lesions. The minimum peritumoral and tumoral ADC values and peritumoral and tumoral ADC ratios were not statistically significant between these groups. In conclusion, peritumoral ADC gradient values can be powerful and useful tool in the diagnosis of solitary brain metastasis and glioblastoma multiforme.
