Zehirlenmeler, etkene ve hastaneye başvuru süresine
bağlı olarak ciddi sonuçlar doğurabilmektedir. Bu çalışmanın amacı, bir üçüncü
basamak acil servisine başvuran zehirlenme vakalarına dair kaydedilen verileri
derleyip, demografik, etiyolojik ve klinik özellikleri ile prognozlarını retrospektif
olarak inceleyerek değerlendirmektir. Eskişehir Osmangazi Üniversitesi (ESOGÜ)
Tıp Fakültesi Acil Servis’e 21.02.2010 - 14.10.2015 tarihleri arasında başvuran 18 yaş
ve üzeri zehirlenme vakaları çalışmaya dahil edilmiştir. Dosya arşivinde yapılan
tarama sonucu kayıtlarına ulaşılan 839 zehirlenme vakasının verileri kaydedildi.
Zehirlenmelere en sık yol açan etkenlerin ilaçlar (%55,4) ve karbonmonoksit (%24,5)
olduğu saptandı. İlaç zehirlenmeleri içerisinde en sık çoklu ilaç alımlarına (%46,8)
rastlandı. Daha sonra en sık karşılaşılan etken maddeler, duygudurum düzenleyici
ilaçlar (%22,9) ve ağrı kesiciler (%16,3) idi. Zehirlenmelerin %54,4’ü intihar
girişimleri sonucu meydana gelmekteydi. Zehirlenmelerin azaltılması için toplumun
eğitilmesi, reçetesiz ilaç satışına kısıtlama getirilmesi, kimyasal maddelerin
kullanımında dikkatli olunması gibi önlemler alınmalıdır. Hastaların ilaç temin etmek
için sık başvurdukları aile hekimleri, akılcı ilaç kullanımı ilkelerine dikkat etmelidir.
Ayrıca aile hekimleri, hastalarının ruhsal durum muayenelerine gereken önemi verip
intihara eğilimli hastaların psikiyatrik destek almalarını sağlamalıdır.
Poisonings can cause
serious outcomes depending on the agent and the time passed before admission to the
hospital. The aim of this study is to review the demographic, etiological and clinical
features and prognosis of patients with poisoning cases referred to a tertiary emergency
department by studying retrospectively. Patients equal or above 18 years from
poisoning cases applied to Emergency Service of Eskişehir Osmangazi Üniversitesi
(ESOGU) Medical Faculty between 21.02.2010 - 14.10.2015 were included in the
study. The data of 839 poisoning cases that obtained by scanning in the file archive
were recorded. Drugs (55.4%) and carbon monoxide (24.5%) were the most common
causative agents of poisonings. Among the drug poisonings, multiple drug intake
(46.8%) was the most encountered. After that, most encountered drugs were mood
regulator drugs (22.9%) and pain relievers (16.3%). 54.4% of the poisonings were
suicide attempts. Precautions such as educating the community, restrictions on the sale
of drugs without a prescription, and taking caution in the use of chemical substances
should be taken to reduce poisonings. Family physicians, oftenly appealed by patients
to provide medicines, should pay attention to the principles of rational drug use. In
addition, family physicians should give attention properly to mental health
examinations and should ensure that patients with suicidal thoughts receive psychiatric
support.
