Behçet hastalığı (BH) geniş
bir klinik spektruma sahip, etyolojisi bilinmeyen multisistemik inflamatuar bir
hastalıktır. Prevalansı en yüksek ülke Türkiye'dir. Çalışmamıza Ağustos 2015-Ocak
2016 tarihleri arasında başvuran, Uluslararası Çalışma Grubu’nun kriterlerine göre
tanı almış 141 hasta(70 E %49,6 ;71 K %50,4) dahil edilmiştir. Ortalama tanı yaşı
33,03±10,55(32,76±11,62 yaş erkek ve 33,30±9,45 yaş kadın) idi ve her iki cinsiyet
arasında anlamlı fark yoktu. Hastaların %15.6’sında aile öyküsü vardı. Hastalığın en
sık başlangıç semptomu oral aftöz ülser(%70,9), genital ülser(%12,8) ve üveit(%7,1)
idi. En sık klinik bulgu oral aftöz ülser(%100), genital ülser(67,4) ve papülopüstüler
lezyonlardı(%67,4). Papülopüstüler lezyonlar(%57.7 K, %77.1 E), üveit(%16.9 K,
%38.6 E) ve derin ven trombozu (DVT) (%12.7 K, %37.1 E) erkeklerde daha sık
görüldü(p<0,05). Diğer klinik bulgularda cinsiyetler arasında anlamlı fark yoktu. EN
olan hastaların %48’inde herpes labialis görülüyordu, aralarında istatistiksel açıdan
anlamlı bir ilişki vardı(p=0,018). Göz tutulumu; gençlerde(ortalama yaş 39,64±9,74),
erkeklerde(%16.9 K-%38.6 E), ve sigara içenlerde(%19.8-%44,4) daha sık izlendi ve
genellikle bilateraldi(54%). Sigara içenlerde artrit daha sık izlendi(%48,9).
Trombofilebiti olanlarda DVT sıklığı anlamlı derecede yüksekti(p=0,002). HLA-B51
bakılan 58 hastanın 31 tanesi pozitifti ve bu grupta sık görülen bir klinik bulguya
rastlanmadı. Paterji testi bakılan 119 hastanın %62,2’si pozitifti ve bu grupta da sık
görülen bir klinik bulguya rastlanmadı.
Behçet's
disease (BD) is a multisystemic inflammatory disorder of unknown origin and with a
wide spectrum of clinical presentations. The highest prevalence is seen in Turkey.
141 Behçet patients (70 M %49.6, 71 F %50.4) diagnosed according to the
International Study Group criteria who applied to rheumatology department in
August 2015-January 2016 were included in the study. The mean age of diagnosis
was 33,03±10,55 (32,76±11,62 years in male and 33,30±9,45 years in females).
There was no significant difference between both sexes. 15.6% of the patients had
the family history. The most common initial presenting manifestation of the disease
was oral aphthous ulcer which was seen in 70.9% of the patients, followed by genital
ulcer (12.8%) and uveitis (%7.1). The leading clinical features were oral aphthous
ulcers (100%), followed by genital ulcers (67.4%) and papulopustular lesions
(%67.4). Papulopustular lesions (%57.7F, %77.1M), uveitis (%16.9F,%38.6M) and
deep vein thrombosis (%12.7F, %37.1M) were more common in males (p<0,05).
There were no significant differences between both sexes in other clinical findings.
HSV infection was found 48% of patients who have erythema nodosum (EN). There
was a significant association between only HSV infection and EN (p=0.018). Ocular
involvement was more frequent in young (mean age 39,64±9,74), male (F%16.9-
M%38.6), and smokers(%19.8-%44,4) and it was usually bilateral (54%). Arthritis
was more common in smokers (%48,9). The incidence of DVT in patients with
thrombophlebitis was significantly higher (p=0.002). HLA-B51 was positive in 31 of
58 patients (%53.4) and there were no frequent clinical signs in this group. 62.2% of
the 119 patients who tested pathergy were evaluated as positive and there were no
frequent clinical signs in this group.
