Subaraknoid kanama (SAK) mortalite ve morbidite oranı yüksek olan
bir inme çeşididir. SAK sonrası en çok karşılaşılan kognitif bozukluklar
hafıza, yönetsel işlevler ve konuşma fonksiyonlarıyla ilgilidir. Bu işlevler
genel olarak medial temporal lobun hipokampüs kısmı ve frontal lob
bölgeleri tarafından kontrol edilmektedir. SAK dışındaki durumlarda izole
ortam koşullarının bu fonksiyonlar üzerine olumsuz, zenginleştirilmiş
ortam koşullarının ise olumlu etkileri olduğu bilinmektedir. Günümüzde
subaraknoid kanamalı hastalar izole ortam ile benzerlikleri bulunan
koşullarda tutularak tedavi edilmektedir. Birçok mikroRNA sinaptik
plastisiteye firekt ya da dolaylı yoldan etki yapmaktadır. Bu mikroRNA’ların
başında miR-132, miR-134 ve miR-138 gelmektedir. Bu nedenle
subaraknoid kanama sonrası farklı ortam koşullarında tutulan hayvanların
dokularındaki miR-132, miR-134 ve miR-138 ekspresyon seviyelerinde
farklılıklar olabileceği öngörülmektedir.
Deneyde sisterna magna içerisine çift enjeksiyon yöntemi ile
subaraknoid kanama modeli oluşturulan sıçanlar 7 ve 14 günlük ayrı
gruplar halinde farklı ortam koşullarında tutulduktan sonra plazma, frontal
lob ve hipokampüs örnekleri toplandı. Bu dokulardaki miR-132, miR-134
ve miR-138 seviyeleri, uygun primerler kullanılarak real-time PCR yöntemi
ile belirlendi. Elde edilen veriler istatistiksel olarak değerlendirildi.
Kısa dönem ve uzun dönem farklı ortam koşulları ile SAK oluşumunun
frontal lob, hipokampüs ve plazma dokularında seçilen mikroRNA
seviyeleri üzerinde farklı etkileri olduğu görüldü. SAK sonrası kısa
dönemde alınan örneklerin hepsinde bütün mikroRNA seviyelerinin
yükseldiği görülürken, uzun dönemde bu seviyelerin düştüğü tespit edildi.
İstisna olarak hipokampüs dokularındaki miR-134 ve miR-138
seviyelerinin uzun dönemle kısa dönem arasında farklılık göstermediği
belirlendi. Frontal lob dokularında mikroRNA seviyeleri değişkenlik
gösterdi. SAK sonrası kısa dönem zenginleştirilmiş ortamda tutulan
hayvanların frontal loblarında üç mikroRNA seviyesi de anlamlı ölçüde
azalırken, uzun dönem zenginleştirilmiş ortamda bütün mikroRNA’ların
seviyelerinin anlamlı ölçüde yükseldiği görüldü. SAK sonrası izole ortamda
tutulan hayvanların frontal loblarında ise zenginleştirilmiş ortamın aksine
kısa dönemde artış uzun dönemde düşüş görülmektedir. Frontal lobda SAK
sonrası 7 günlük izole ortam koşullarında zengin ortama göre sinaptik
yeniden yapılanmanın daha yoğun olduğu, buna karşın SAK sonrası 14
günlük zenginleştirilmiş ortam koşullarının izole ortam koşullarına göre
sinaptik yeniden yapılanmaya ve iyileşmeye daha çok katkı sağladığı
görülmüştür.
Subarachnoid hemorrhage (SAH) is a stroke type with a high
mortality and morbidity rate. The most common cognitive impairments
after SAH are related to memory, executive and speech functions. These
functions are generally controlled in the frontal lobe and hippocampal
region of the medial temporal lobe. It is known that social isolation
conditions affect negatively while enriched environment have positive
effects on these functions in healthy individuals. Nowadays, after SAH
patients receive treatment at the social isolation like environments during
recovery period. Many microRNAs cause direct or indirect effects on
synaptic plasticity. The most important of these microRNAs are miR-132,
miR-134 and miR-138. In this thesis study, it was aimed to observe the
changes of miR-132, miR-134 and miR-138 levels in the tissues of animals
which were kept in various environmental conditions after subarachnoid
hemorrhage.
Rats in control groups and cisterna magna double-hemorrhage model
of subarachnoid hemorrhage group were kept in different environmental
conditions for 7 and 14 days. Then, animals were sacrificed and blood,
frontal lobe and hippocampus samples were collected. miR-132, miR-134
and miR-138 levels in these tissues were measured using specific primers
for these microRNAs and real-time PCR. Statistical differences between
different groups were determined by Student's two-tailed t-test.
It has been determined that living short term and long term in
different environmental conditions and SAH formation have different
effects on selected microRNA levels in frontal lobe, hippocampus and
plasma tissues. All of the samples taken from the short term after SAH
group showed that all the microRNA levels were increased, whereas those
from the long term group were decreased. As an exception, miR-134 and
miR-138 levels in hippocampal tissues do not differ between the long term
and short term groups. The levels of microRNAs in the frontal lobe tissues
are highly variable. While the levels of all three microRNAs in the frontal
lobes of the animals who were maintained in the short term enriched
environment after SAH were significantly reduced, microRNA levels of
animals from the long term enriched environment group were significantly
increased. However, in social isolated environment microRNA levels were
increased in short-term and decreased in long-term conditions as opposite
to the enriched environment. In the frontal lobe, it was observed that the
synaptic reconstruction was more intense in the 7-day social isolation
condition compare to the enriched environment. On the other hand, 14-
days enriched environment conditions after SAH showed more positive
effect on the synaptic reconstruction and healing compare to the isolated
conditions.
