Çalışma, Dünya Sağlık Örgütü tarafından geliştirilen ve MPOWER (İzle, Koru, Öner, Uyar, Yasakla, Vergilendir) olarak kısaltılan topluma yönelik sigara karşıtı müdahale uygulamalarının tüm yaş gruplarında “sigara içme sıklığı”, “sigara bırakma hızı” ve “günlük içilen sigara sayısı” değişkenleri üzerine etkililiğini değerlendirmek amacı ile Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Halk Sağlığı Anabilim Dalı’nda Ocak 2016-17 tarihleri arasında yürütülen bir meta analiz çalışmasıdır. Çalışmada “smok*, tobacco, cigarette*” anahtar kelimeleriyle “Medline, Web of Science, Scopus, Cochrane-Library” veri tabanlarından POWER kriterlerinin uygulama alanlarına ilişkin tarama yapıldı. Makaleler araştırma ekibi tarafından başlık, özet, tam metin, analize dâhil etme kriterleri ve kalite değerlendirilmesinden geçirilerek meta analize alındı. Çalışmalarda 6 aydan kısa ve 6 ay ve daha fazla süre takipli müdahaleler kısa ve uzun dönem olarak değerlendirildi. Analizler öncesi heterojenite testi yapıldı, test sonucu p<0.05 ise rastgele, p≥0.05 ise sabit etki modeli kullanıldı. Ana değişken olarak sigara bırakma hızını kullanan P (koru) ve O (öner) kriterlerine ait uygulamalar en uzun takip döneminde erişkinlerde sigara bırakma hızını %39 artırırken [RR:1.39(1.23-1.57)], uygulamaların adölesanlarda sigara bırakma hızı üzerine etkili olmadığı belirlendi [RR:1.13(0.90-1.42)]. Erişkinlerde sigara bırakma hızı üzerine en etkili uygulama “NYKT temelli sigara bıraktırma çalışmaları” idi. MPOWER uygulamalarının bir bütün olarak sigara kullanımı ile ilgili değişkenler üzerine farklı düzeylerde etkili olduğu ve bu etkililiğin farklı yaş gruplarında ve farklı zaman dilimleri içerisinde değişkenlik gösterdiği belirlendi. Sigara karşıtı mücadelede etkililiği kanıtlanmış, uygun maliyetli, iyi planlanmış ve iyi denetlenen sigara karşıtı uygulamaların, tüm ülkelerde, en yüksek seviyede yürürlüğe konması halk sağlığının korunması adına büyük önem taşımaktadır.
The study is a meta-analysis study that performed on Eskisehir Osmangazi University Faculty of Medicine, Department of Public Health between January 2016-2017 on the purpose of assessment of effectiveness of community oriented anti-smoking implementations enhanced by World Health Organization (WHO) and under the name of MPOWER (Monitor, Protect, Offer, Warn, Enforce, Raise) on "smoking prevalence rate", "quit rate" and "number of cigarettes smoked per day" variables in all age groups. In the study, several searches have been made with "smok*, tobacco, cigarette*" keywords from “Medline, Web of Science, Scopus, Cochrane-Library” databases related to subdomain of POWER criteria. The articles are attached to meta-analysis after evaluation on quality assessment, inclusion criteria, full text, abstract, title by research team. In the study, interventions which has less than 6 month and 6 month and over follow up period evaluated as short term and long term respectively. Before the analysis, heterogeneity test was made. If the test resulted p<0.05 random effects model were used, whereas the result p≥0.05 fixed effect model were used. It is stated that quit rate were raised to %39 [RR:1.39 (1.23-1.57)] on adults in longest survey term by implementations which uses quit rate as main variable belongs to P and O criteria, whereas on adolescent, these implementations have no effect upon quit rate [RR:1.13(0.90-1.42)]. The most effective implementation on quit rate of adults was "NRT based smoking cessation studies". In the light of these results it is stated that, MPOWER implementations have effect on different levels upon variables which related to smoking and also this effectiveness vary from age groups to period. Bringing into force of anti-smoking implementations, which well-controlled, well-planned, cost-effective and effective on anti-smoking campaign on all countries have great importance on prevention of public health.
