Aferez trombosit süspansiyonu; daha az lökosit içermesi, daha az sayıda donör gerektirdiğinden enfeksiyon riskinin az olması, havuzlama yapılmadığından bakteriyel kontaminasyon riskinin az olması, yeterli miktarda konsantre ürün elde edilmesi ve febril non-hemolitik transfüzyon reaksiyonlarını azaltmasıyla güvenli bir yöntemdir. Aferez donasyonu genel olarak güvenli bir işlem olarak kabul edilmekle birlikte işlemin koagülasyon,fibrinolizis ve trombosit aregasyonu üzerine etkileri net değildir. Ekstrakorporal dolaşımın olduğu hemodiyaliz ve açık kalp cerrahisinde olduğu gibi aferez donörlerinde de donör kanının yapay yüzeyler ile teması ve santrifüj sırasında eksternal kuvvetlere maruz kalması trombosit ve/veya lökositleri aktive edebilir. Çalışmamızda trombosit aferezi yapılan sağlıklı donörlerde işlem öncesi, işlemden hemen sonra, 1 gün sonra ve 1 hafta sonra olmak üzere trombosit-nötrofil, trombosit-monosit, trombosit-lenfosit agregat düzeyleri, trombosit aktivasyon göstergesi olarak CD62P (P-selektin) düzeyleri ve hemostatik sistemin aktivasyon göstergesi olarak protrombin fragman 1+2 düzeyleri çalışılarak aferez işleminin donörlerde ortaya çıkaracağı koagülasyon sistem değişikliklerinin araştırılması amaçlanmıştır. Çalışmamızda trombosit-granülosit, trombosit-lenfosit ve trombosit-monosit agregat düzeylerinde trombosit aferezi sonrası azalmayı takiben 1. gün ile 1. hafta sonunda tekrar istatistiksel olarak anlamlı bir artış olması işlemin donörlerde prokoagulan bir duruma yol açtığını düşündürmektedir. Hemostatik aktivasyonun in vivo moleküler göstergesi olan protrombin fragman 1+2 ve trombosit aktivasyon göstergesi olan P-selektin düzeylerinde trombosit aferezi sonrasında azalma saptanmış, daha sonra ise artış olmakla birlikte bazal değere ulaşamamış ve istatistiksel olarak anlamlı bulunmamıştır. Sonuç olarak, trombosit aferezi sonrası trombosit-granülosit, trombosit-lenfosit ve trombosit-monosit agregat düzeylerinde saptanan artış donörlerdeki hiperkagülabl durumu desteklemektedir.
Apheresis platelet suspension is safer compared to platelet suspension, due to containing less leukocyte, low risk of infection because of being taken from single donor, low risk of bacterial infection because of not being pooled, obtaining a sufficent amount of concantrated product and causing less febril nonhemolytic transfusion reaction. As plateletpheresis donation is commonly accepted as a safe procedure, the effect on the coagulation system, fibrinolysis and platelet aggregation is not known well. Similar to hemodialysis and open heart surgery procedure including extracorporeal blood flow, the contact of blood with artificial surfaces and also external forces during centrifugation may activate platelets and/or leukocytes. In our study we examined the effects of plateletpheresis on the coagulation system of donors, before, immediately after, 1 day after and 1 week after the process by evaluating the levels of platelet-lymphocyte, platelet-monocyte, platelet-neutrophil aggregates, CD62P ( P selectin) levels as thrombosis activation indicator and prothrombin fragment levels as the hemostatic system activation indicator. In our study, there was a reduction in the platelet-granulocyte, platelet-monocyte, platelet-lymphocyte aggregate levels after plateletpheresis, there was also a statistically significant increase on 1 day and 1 week later, suggesting that the process causes a procoagulant state in the donors. There was a reducion in the prothrombin fragment levels, as an in vivo moleculer indicator of the hemostatic activation and P-selectin levels, as a pletelet activation indicator, after plateletpheresis. Although there was an increase shown later, the values did not reach the basal levels and was not statistically significant. As a result the increase detected in the platelet-granulocyte, platelet-monocyte, platelet-lymphocyte aggregate levels after plateletpheresis support a hypercoagulable state in donors.
