Çalışmamızda kontinyu
spinal anestezi (KSA) altında operasyona alınmış ortopedik vakalarda duyusal ve
motor blok seviyeleri tespiti ile spinal blok kalitesini ortaya koymak ve postspinal
baş ağrısı (PSBA) sıklıklarını tespit etmeyi amaçladık. Bu çalışma, Ocak 2015-
Şubat 2016 tarihleri arasında, KSA altında ortopedik alt ekstremite cerrahisine alınan
55-92 yaş aralığındaki, ASA II, III, IV 18 erkek, 15 kadın toplam 33 hastanın
dosyalarının incelenmesi ile gerçekleştirildi. Anestezi kayıtlarından ve servis takip
formlarından yapılan incelemelerde hastaların PSBA sıklığı, bulantı-kusma, kulak
çınlaması, ışığa duyarlılık, sese duyarlılık oranları ve hemodinamik ölçümleri ile
motor ve duyusal blok seviyesi kayıtları tespit edildi. Elde edilen verilerin
istatistiksel anlamlılığı Pearson Ki-Kare testi, Kruskal Wallis H testi ve Mann
Whitney U testleri ile değerlendirildi. P>0.05 anlamsız, P<0.05 anlamlı kabul edildi.
Spinocath, B. Braun® seti kullanılarak 27-gauge (G) iğne üzerinden 22G kateter
yerleştirilerek KSA uygulanmış olan hastalara ilk doz 7,5 mg % 0.5 hiperbarik
bupivakain ve istenilen blok seviyesine ulaşılana kadar idame doz uygulanmış.
Hastaların tamamında 30-390 dakika süresince yeterli anestezi için uygun seviye
motor ve duyusal blok sağlanmıştır. Hastaların %48,52’sinde postoperatif ilk 24
saatte bulantı-kusma şikayeti görülmüştür.Bulantı-kusma şikayeti gelişen hastaların
%57,9’una postoperatif dönemde opioid verildiği görüldü. Hastaların hiçbirinde
PSBA’na rastlanılmamıştır. Sonuç olarak bizim çalışmamızda incelenen veriler
doğrultusunda iğne üzeri kateter tekniği (İÜKT) ile uygulanan KSA; PSBA sıklığı,
motor ve duyusal blok kalitesi açısından başarılı ve yeterli bir tekniktir denilebilir.
Ancak bu konuda kanıta dayalı yaklaşımlar geliştirebilmek için daha fazla sayıda
olgunun incelendiği çalışmalar gerekmektedir.
In our study, we aimed to identify spinal block
quality from sensorial and motor block levels and post-dural puncture headache
(PDPH) frequencies of patients who underwent orthopedic surgery under continuous
spinal anesthesia (CSA). This study, was conducted after analysis of the records of
18 male and 15 female total 33 patients aged 55-92 years, ASA physical status
classification II, III, IV who underwent orthopedic lower limb surgery under CSA
between January 2015-February 2016. Nause and vomiting, tinnitus, photophobia,
phonophobia and PDPH rates and hemodynamic measurements motor and sensorial
block levels of patients were obtained from the anesthesia recordings and service
forms. Statistical significance of the data was evaluated with the Pearson Chi-Square
Test, Kruskal Wallis H Test and Mann Whitney U Test. P>0.05 accepted as
insignificant and P<0.05 accepted as significant. Continue spinal anesthesia was
performed with Spinocath, B. Braun® set that 22-gauge (G) spinal catheter was
inserted to patients over a 27G needle and first dose of 7,5 mg 0.5% hyperbaric
bupivacaine was administered and additional doses was administered until the
appropriate sensorial block level reached. In all patients appropriate motor and
sensorial block level were reached for adequate anesthesia during 30-390 minutes.
Nause and vomiting was seen in 48,52% of the patients in postoperative first 24
hours. It was seen that in an amount of patients (57,9%) that nause and vomiting was
seen, were treated with opioids in postoperative period. Post-dural puncture headache
was not seen in any of the patients. In conclusion of our study ;in terms of motor and
sensorial block quality CSA performed with catheter over needle technique is a
succesfull and adequate technique. Unless it is necessary to do studies which
examine more records of patients to develope evidence based practice.
