Prematür over yetmezliği (POY), 40 yaş altı kadınlarda FSH (Follicle Stimulating Hormone) değerlerinin 40 IU/L ve daha üzerinde olduğu, en az 4 ay amenorenin görüldüğü, ovaryan foliküllerin erken azalması ile karakterize, folikül oluşumunu kontrol eden birden fazla genin etkili olduğu, kompleks heterojenik klinik bir hastalıktır. Kadınların %1'inde görülen POY tablosunun etiyolojisi tam olarak aydınlatılamamış olsa da, literatür verileri güçlü bir genetik bileşeni olduğunu öngörmektedir. POY etiyolojisinin %20’sini ailesel genetik nedenler, %10’unu X kromozom anomalileri oluşturmaktadır. Son yıllarda gelişen teknolojik analiz metotları ile X kromozomunun yanı sıra AIRE, DAZL, FOXL2, FSHR, GALT ve TGF-I gen ailesi üyelerinden GDF9, INHJ gibi otozomal kromozomlarda lokalize genlerdeki değişikliklerin de POY’da rolü olduğu düşünülmektedir.
Kromozom analizleri 5-6 Mb’dan daha büyük yapısal kromozom anomalilerini ortaya koyabilirken daha küçük kopya sayısı değişimlerini tüm genom genelinde belirlemeyi olanaklı kılan mikroarray yöntemi son yıllarda POY olgularında genetik temelin ortaya konmasında aktif olarak kullanılmaktadır. İnsan genomunun yüksek çözünürlükle taranması ve kopya sayısı değişikliklerinin (CNV) tanımlanması, klinik olarak anlamlı lokusların tespit edilmesine yol açmıştır. Mikroarray yöntemi ile spesifik olarak değerlendirilebilecek anomalilerin saptanması, bu hasta grubuna özgü tanı kitlerinin oluşturulmasına veri sağlayacaktır. Ayrıca olguların ailelerine ve risk altındaki aile bireylerine genetik danışma verilerek bilgilendirilebilmesi ve yönlendirilebilmesi mümkün olacaktır. Bu çalışma, POY etiyolojisinde rol oynayan ve daha önce literatürde tanımlanmış genetik faktörlerin durumunun ortaya konması açısından da bir veri oluşturacaktır. Bu hedefler doğrultusunda POY olgularında yüksek rezolüsyonlu mikroarray analizi ile CNV açısından tüm genom taranarak aday genlerin belirlenmesi, genetik temelin ortaya konması ve takibinde fonksiyonel çalışmaların yapılması amaçlanmıştır.
Çalışmamıza klinik ve genetik değerlendirmeleri yapılan, yaş ortalaması 30,4±6,36 olan, sekonder amenoresi bulunan toplam 55 POY olgusu dâhil edilmiştir. Klinik genetik değerlendirme sonrası sendromik olmayan ve normal kromozom kuruluşuna sahip olan olgularda, Frajil X premutasyon değerlendirmesi için FMR1 geni fragman analizi gerçekleştirilmiştir. Tüm olgularda normal tekrar dizisi saptanmıştır. Çalışmamızın son aşamasında olgu örneklerinde Agilent® 8x60K platformu kullanılarak aCGH analizleri yapılmış, %40 olguda (22/55) 15 farklı CNV tespit edilmiştir. Saptanan CNV’lerin %60’ının (9/15) delesyon, %27'sinin (4/15) duplikasyon, %13’ünün (2/15) ise aynı bölgede hem delesyon hem de duplikasyon olduğu gözlenmiştir. Saptanan anomalilerden kromozom 15q26.3 ve kromozom Xp22.33 bölgelerine ait değişiklikler FISH analizleri ile doğrulanmıştır. Çalışmamızda %12,73 oranında değişikliği saptanan ASMTL, P2RY8, SHOX genleri ile değişikliği %1,82 sıklıkla saptanan MSX1 genleri literatürde de aday genler olarak bildirilmiştir. Ayrıca çalışmamızda oosit ve folikülogenez sürecinde fonksiyon gösterdiği bilinen, değişikliklerinde oogenezin olumsuz etkilenmesi, germ hücrelerinin deplesyonu, ovarian foliküllerin yapısal ve fonksiyonel olarak düzensizlikleri sonucu infertilitenin gözlendiği bildirilen PCSK6, NR4A2, NRG1, SPIRE2, PDPK1, KLHL4, SIRT1 genlerini içeren lokuslarda değişiklikler gözlenmiştir. Sonuç olarak POY’un etyopatogenezinde rol oynayabileceği bildirilen aday genler çalışmamızda da ortaya konmuş, bunların yanı sıra ileri analizlerin yapılması gereken yeni genler de literatüre kazandırılmıştır.

Premature Ovarian Failure (POF) is a complex heterogeneous clinical disease that may be affected by the multiple genes that control follicle formation and is characterized by an early loss of the ovarian function. POF occurs in 1% of women and is defined as amenorrhoea for at least 4 months before age 40, with an FSH serum level is than 40 mIU/ml or higher than.
Although the underlying etiology of POF has not been fully understood yet, previous reports predicts that POF is a strong genetic component. In the etiology of POF, the incidences of the involvement of familial genetic causes and X chromosome abnormalities are about 20% and 10%, respectively.
Recently, with developing technology analysis methods and X chromosome along with, alterations of the genes such as AIRE, DAZL, FOXL2, FSHR, GALT and the members of TGF-I gene family comprising GDF9 and INHJ localized on autosomal chromosomes play important roles in POF.
The resolution of conventional cytogenetic analysis is approximatelly 5-6 Mb and can only detect majör chromosome abnormalities. Array comparative genomic hybridization (a-CGH) analysis is able to detect submicroscopic chromosomal rearrangements with a higher genomic resolution. Recent studies have shown usefulness of a-CGH in the detection of whole genome aberrations simultaneously. They have also suggested associations between copy number variations (CNVs) and different common disorders and rare diseases. Thus, exploring the role of CNVs in many traits and diseases becomes essential. High definition scanning of the human genom and the identification of CNVs has led to the identification of clinically significant locus.
Therefore, in the current study, we aimed to reveal the presence and the prevalence of CNVs, using a-CGH analysis on the entire genome, in a cohort of 55 patients with POF.The ages of the patients varies in between 19 to 39 years (30,4±6,36 SD). Following detailed clinical examinations, non-syndromic POF cases with normal karyotypes were included to the study. The DNAs from peripheral blood samples of the cases were extracted and FMR1 gene premutation analysis was performed by using the Fragile X-PCR technique. No expanded CGG repeats was revealed in the cases. Then we performed a-CGH analysis by using high-resolution Agilent oligonucleotide arrays in a total of 55 POF. Totally, 15 different CNVs were seen in 40% (22/55) of the cases. Of these CNVs, 60% (9/15) were deletions whereas 27% (4/15) were duplications and 13% were both deletions and duplications of the same region. Most of the a-CGH detected alterations were confirmed by FISH analysis.
The genes including ASMTL, P2RY8, SHOX on chromosome Xp and MSX1 gene on chromosome 15q have already been reported as candidate genes in the development of POF. The alteration frequencies of these chromosomes in our study were 12% and 1.82%, respectively. Therefore the results of our study are in agreement with the previously reported data.
Moreover, it is known that the genes including PCSK6, NR4A2, NRG1, SPIRE2, PDPK1, KLHL4, and SIRT1 have functions during oogenesis and foliculogenesis processes. The alterations in these genes causes oogenesis abnormalities, germ cell depletions and infertility because of structural and functional disorders of ovarian follicules. In this study, CNVs in the loci comprising these genes were detected in the samples of some cases. Statistically significant differences were seen when CNVs identified in our study were compared with CNVs reported in phenotypically normal individuals from control populations from the web site Database of Genomic Variants. Determined with microarray method and detection of anomalies will provide data that can be evaluated as the creation of specific diagnostic criteria to this patient group. Also given to the families of patients can be informed and is directed genetic solidarity will be possible in families at risk. This work involved in the etiology of POF and will form a previously defined data in the literature to reveal the situation in terms of genetic factors. Identification of high resolution genome wide microarray analysis in terms CNV candidate genes were screaned in accordance with objectives set out in the POF cases of the genetic bases and be made functional in monitoring studies are targeted.
In conclusion, the alterations of the previously reported loci including novel genes involved in etiopathogenesis of POF were also revealed in our study. Besides, new candidate genes that need further analyses in larger cohorts of POF women were detected.

KML, pluripotent kök hücrenin myeloid progenitör hücrelerde artmış proliferasyon ve azalmış apoptozu ile karakterize klonal bir bozukluktur. Kromozom 9 ile 22 arasında karşılıklı translokasyon sonucu BCR/ABL füzyon geni oluşmaktadır. ABL ve BCR genleri üzerindeki kırılma noktası değişebilmektedir. Bu nedenle değişik moleküler kütlelere sahip füzyon protein kinaz ürünleri meydana gelmektedir. Hastalarda BCR/ABL gen füzyonunun yanı sıra ABL ve BCR genlerinde farklı yapıda büyük delesyonlar ve mutasyonlar rapor edilmiştir. Saptanan delesyonların, hastanın prognozu ile ilişkili olduğu gösterilmiştir. BCR/ABL geni üzerindeki mutasyon tiplerinin belirlenmesi, tanı, tedavi ve tedaviye yanıtta önemlidir. KML ve ALL’li hastalarda direncin erken dönemde tanımlanması ve optimal tedavi şeklinin belirlenmesi açısından önemi bilinmektedir. Bu çalışmada, Ph+ olan elli dokuz hasta Minisekans metoduyla BCR/ABL füzyon genindeki en sık görülen 6 mutasyon taranmıştır. Yirmi dokuz Ph+ KML hastası dirençli grup, yirmi Ph+ KML kontrol ve 10 Ph+ ALL hasta grubu, negatif kontrol olarak Philadephia (-) DNA örneği kullanılmıştır. Nokta mutasyon analizi için olguların kan ve/veya kemik iliği örneklerinden izole edilen BCR/ABL füzyon geni DNA’sı üzerinde toplam 6 mutasyon incelenmiştir. Bir hastada G250E (%1.69) mutasyonu saptanmış, Y253H, T315I, E255K, F317L ve M351T mutasyonları ise saptanamamıştır. Hiç mutasyonu olmayan hastaların oranı %98.30 (58) olarak bulunmuştur. İlaçlara direncin moleküler temelinin anlaşılabilmesi için özellikle BCR /ABL füzyon genindeki mutasyonların daha ayrıntılı ve geniş populasyonlarda araştırılması gerekmektedir. KML ve ALL’li hastalarda mutasyon tipinin belirlenmesiyle, mutasyona uygun tedavi şeklinin planlanmasına katkı sağlanacağı düşünülmektedir.
CML, of pluripotent stem cells, at myeloid progenitör cells of increased poliferation and deceased apoptosis is characterized by a clonal disorder. Of BCR/ABL fusion gene the reciprocal translocation of chromosomes 9 and 22 is caused. Of BCR and ABL genes breaking point can vary on. Therefore kinase fusion protein products having different molecular mass is formed. In patients with BCR/ABL of gene fusion as well as of BCR and ABL genes in different structures large deletions and mutations have been reported. Deletions detected, has been shown to be related to patient prognosis. BCR/ABL gene mutations on the identification of the types, diagnosis, treatment and response to treatment is important. In CML and ALL patients, of the resistance diagnosed early identification and determination of the optimal treatment for planning plays an important role. In this study, fifty-nine patients with Ph+, BCR/ABL fusion gene by the method of minisequence most common six mutations were screened. Twenty-nine Ph+ CML patients resistant group, twenty Ph+ CML patients control group and ten Ph+ ALL patients was used. For point mutation analysis of patients blood or bone morrow sample isolated from BCR/ABL fusion gene on the total six DNA mutations were examined. Mutations were detected in one of fifty-nine patients (%1.69), this mutation is G250E. Y253H, T315I, E255K, F317L and M351T mutations were not detected. Ninety-eight percent of patients without the mutation. For an understanding of the molecular basis of drug resistance, particularly BCR/ABL fusion gene mutations in large populations need to be studied in more detail. In patients with CML and ALL with determining the type of mutation, mutations that contribute to the planning of appropriate treatment is considered.

GİRİŞ: Melanositler melanin üreten hücreler olup, bu hücrelerin malign değişimi ile malign melanom oluşur. Dünya sağlık örgütüne göre her yıl 132000 yeni tanı malign melanom vardır ve bu sayının, ozon tabakasının UV’ye karşı koruma özelliği azaldıkça artacağı düşünülmektedir. AMAÇ: Malign melanom ve melanositik nevüslerin histopatolojik olarak ayırımını yapmakta bazı zorluklar söz konusu olup, bazen tanı aşamasında kararsızlık yaşanmaktadır. Biz çalışmamızda malign ve benign olgular arasındaki genetik değişiklikleri ortaya koyarak, klinikte tanı, tedavi ve takip süreçlerinde FISH yönteminin etkisini göstermeyi amaçladık. GEREÇ VE YÖNTEM: Histopatolojik olarak tanı konmuş 18 malign melanom ve 24 melanositik nevüs olgusuna ait, deparafinize edilmiş doku preparatları, FISH yöntemi ile, CCND1, MYB, RREB1, ve sentromer 6 bölgelerine ilişkin problar kullanılarak, kopya sayısı değişiklikleri incelenmiştir. BULGULAR: Elde ettiğimiz sonuçlara göre 18 melanom olgusundan 15’inde (%83,33) aberasyon saptanmış olup, 5 olguda en az iki aberasyon gözlenmiştir. Melanositik olgulardan 20 tanesi normal olup, geriye kalan 4 olgudan 3’ünde sadece bir aberasyon pozitif değerlendirilirken, geriye kalan bir olguda üç aberasyon gözlenmiştir. SONUÇ VE TARTIŞMA: Malign melanom olgularında en fazla CCND1 ve RREB1 gen amplifikasyonları (%44,44) gözlenmiştir. Malign melanom ve melanositik nevüs olgularında elde ettiğimiz sonuçları karşılaştırdığımızda CCND1, RREB1 ve RREB1/CEP6 açısından anlamlı sonuçlar elde edilmiş, ancak MYB gen delesyonu için istatiksel olarak anlamlı sonuç alınamamıştır. Elde ettiğimiz sonuçlara göre, MYB delesyonu haricinde saptanan diğer aberasyonlar ayırıcı tanı kriteri olarak kullanılabileceğini ve FISH analizi ile aberasyon saptanan melanositik nevüs olgularının, malign melanomla ayrımlarının histopatolojik yöntemlerle zor yapılması ve malign melanoma öncül olmalarından dolayı, bu örneklerin patologlar tarafından tekrar değerlendirilmesi gerektiğini düşünmekteyiz.
BACKROUND: Melanosits are cells which generating melanine and if these cells have
malign mutation, they cause to Malign Melanom. World Health Organization says
there are 132000 new diognasis malignant melanoma determined every year and the
rate going to increase because of decareasing of UV protection of Ozone.
AİM: There is instability between pathologists sometimes, because of some kind of
difficulties of histopathologic discrimination between malignant melanoma and
melanocytic nevi. Thinking of using FISH at diagnosis of malignant melanoma is
useful, we purposed to analize copy number alterations cases of malignant melanoma,
and melanocytic nevi by FISH.
METHODS: We analized 42 preparat deparaffined, included cases of 18 malignant
melanoma, and 24 melanocytic nevi by FISH targeting CCND1, MYB, RREB1, CEP6
region.
RESULTS: As results of our study, 15 cases of malignant melanoma (%83,33) shown
copy number alterations. Five cases of them have least two abnormalities. On the other
hand, of 24 melanocytic nevi cases 20 have no aberations, but one of four cases shown
abnormalities about 3 criters, and remained 3 cases have only single aberations.
CONCLUSION: RREB1 ve CCND1 gene amplifications were the most frequent
abnormalities (%44,44) among our study. We compared the results of malignant
melanoma and melanocytic nevi, and obtained significant inequality for CCND1,
RREB1 and RREB1/CEP6 criters. Although result of MYB gene deletion was no
statistically significant. The cases with melanocytic nevi shown pozitif results by FISH
were diagnosed with properties similiar to malignant melanoma as histopathologycally
and these subtype may turn into malignant melanoma, so we support these cases must
re-analyzed by patologists.

Otizm; yaşamın ilk
dönemlerinde başlayan, sosyal ilişkide ve etkileşimde bozulma, sözel ve sözel
olmayan iletişimde gerilik, tekrarlayıcı ve basmakalıp davranışlarla karakterize,
etkileri yaşam boyu devam eden “Yaygın Gelişimsel Bozukluklar” (YGB)
kapsamında değerlendirilen nörogelişimsel bir bozukluktur. Son yıllarda tüm genom
bağlantı ve asosiasyon çalışmaları, Y kromozomu haricinde tüm kromozomlarda
lokalize 175 kadar lokusun YGB etyolojisinde rol oynayabileceğini göstermiştir.
Olguların % 1-3 kadarında kromozom 15q11-q13 duplikasyonu, % 1 kadarında da
kromozom 16p11.2 ve 22q13 delesyonları gözlenmektedir. Bu bölgelerde lokalize
UBE3A, GABRA5 ve GABRB3 (kromozom 15q11-q13) ve SHANK3 (kromozom
22q13) genleri YGB ile ilişkisi ortaya konan genler arasındadır. Çalışmamızda
Eskişehir bölgesinde YGB tanısı alan, kromozom ve FMR1 geni analizleri yapılan
toplam 80 olguda 15q11’de lokalize UBE3A, 15q12’de lokalize GABRB3 ve
CHRNA7’yi içeren 15q13 mikrodelesyon bölgesi ile 16p11 mikrodelesyon bölgesi
ve 22q13’te bulunan SHANK3 geni mutasyonlarının MLPA yöntemi ile incelenmesi
hedeflenmiştir. Bu amaç doğrultusunda SALSA MLPA P339-A1 (lot1209) ve P343-
C1 AUTĠSM-1 (lot0310) prob kitleri kullanılmıştır. Olgulardan sadece bir tanesinde
16p11.2 delesyonu saptanmış olup görülme sıklığı literatür ile uyumlu olarak
(%1,25) değerlendirilmiştir. Olgumuzda konuşma geriliği, YGB ve belirgin kulak
lobları fenotipi mevcuttur. Çalışmamızda; MLPA yöntemi ile YGB olgularının ilgili
mutasyonlar açısından taranabileceği, YGB olgularında etkin bir genetik danışma
için taramaların gerekli olduğu sonucuna varılmıştır.
Autism; starting at the first stages of life, is charecterized by impared social
interactions and comunications, backwardness of verbal and non verbal
comunications, repetitive and stereotyped behaviors. Effects throughout life within
“pervasive developmental disorder” (PDD) assessed in the context of
neurodevelopmental disorder. In recent years, genome linkage and association
studies showed up to 175 locus that may play PDD ethiology localized to all
chromosomes except the y chromosome. Up to %1-3 chromosome 15q11-q13
duplication, up to %1 chromosme 16p11.2 ve 22q13 deletion of cases had been
observed. Regions in UBE3A, GABRA5 and GABRB3 (chromosome 15q11-q13)
and SHANK3 (chromosome 22q13) genes are among the genes related to PDD. The
diagnosis of PDD in EskiĢehir area total of 80 cases microdeletion regions were
localised 15q11 in UBE3A, 15q12 in GABRB3, 15q13 in CHRNA7, microdelation
in region of 16p11, mutation in the SHANK3 gene located in 22q13 was targeted to
investigate by the method of MLPA. For this purpose, SALSA MLPA P339-A1
(lot1209) and P343-C1 Autism-1 (lot0310) probe kits were used. According to the
incidence of literature (1.25%) 16p11.2 deletion was detected only in one of the
cases. In our case there are delayed speech, PDD and prominent ear lobes phenotype.
Result of our study mutations of PDD cases can be scanned by MLPA method and its
importance in the effective genetic screening of PDD cases was concluded.

Tüm kanserler içerisinde intrakranial tümörler % 1,5 oranında görülmekle beraber tüm
kansere bağlı ölümlerin %2’sinden sorumludur. Meningiomlar primer intrakranial
tümörlerin yaklaşık %20’sini oluştururlar. Yavaş büyüyen tümörler olmakla beraber
çoğunlukla iyi huylu olmalarına rağmen kötü huylu davranış gösterenleri de vardır.
özellikle de kadınlarda 40-60 yaş grubu arasında daha sık görülmektedir. Çocukluk çağı
meningiomları erişkin meningiomlarına göre daha yüksek malignite sıklığına sahiptir.
Meningiomlar meninkslerin bulunduğu her bölgede görülmektedirler.
Atipik ve malign meningiomlarda 1p kaybı sıklıkla tespit edilmekle beraber tümör
progresyonunun ilerlemesinde belirteç olabilir. Malign dönüşümde ilk dikkat edilmesi
gereken kromozomal bölgedir. Kromozom 14q kaybı benign, atipik ve anaplastik
meningiomlarda görülmektedir. Son dönemlerde yapılan çalışmalarda 1p/14q kayıpları
atipik ve anaplastik meningiomlarda oldukça sık gösterilmiştir. Kromozom 1p, 1p/14q
gen delesyonlarının çalışmalarda meningiom prognozu ve progresyonununda önemli rol
oynayabileceği vurgulanmıştır.
Çalışmamızda Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Nöroşirürji Anabilim
Dalı’na başvuran ve Patoloji Anabilim Dalı tarafından histopatolojik olarak incelenerek
“Meningioma” tanısı alan 30 olgu Tıbbi Genetik Anabilim Dalı sitogenetik ve
moleküler sitogenetik laboratuvarlarında çalışılmıştır. Bu olgular histopatolojik
yöntemlerle 26’sı benign, 4’ü atipik meningiom olarak sınıflandırılmıştır. Bu olgularda
EGFR, 1p36 ve 14q genlerine ait değişimler moleküler sitogenetik (FISH) analizlerle
incelenmiştir. Olgularımızın % 20’sinde 1p36 delesyonu, %23’ünde EGFR trizomisi, %3,3’ünde EGFR amplifikasyonu ve tetrazomisi, %13,3’ünde 14q (IGH) delesyonu
saptanmıştır. Gen değişimleri, tümör dereceleri, tümör lokalizasyonları arasında
istatistiksel bir anlam gözlenmemiştir.
Çalışmamız sonucunda meningioma progresyonunda 1p/14q delesyonlarının tümör
derecelendirilmesi ve progresyonu hakkında bir belirteç olarak kullanılabileceği
düşünülmektedir.
Intracranial tumours occurs witj in all cancers with 1,5 % frequency. Besides
intracranial tumours are responsible of two personed among all cancer related daths.
Meningiomas constitute approximately 20% of primary intracranial tumors. ,Although
vast majority is benign and growing slowly there are also malignant once. This type of
tumours are common between fourth and sixth decades, particularly in women. The
childhood meningiomas tend to act more malignant than adulthood meningiomas.
Meningiomas can ocur where meninges exist.
The loss of chromosome 1p which is detected in atypical and malign
meningiomas could be used as a marker for tumour progression. The letter region is an
important side transformation. Loss of 14q was found in benign, atypical and anaplastic
meningiomas. In recent studies loss of 1p/14q were commonly found in atypical and
anaplastic meningiomas.
In our study 30 cases with meningioma were included. Those cases were
classified using histopathological methods: 26 benign,4 atypical meningiomas. Genetic
alterations in EGFR gene, 1p36 and 14 loci were studied using FISH. Deletionsin 1p36,
trisomy of EGFR, amplification and tetrasomy of EGFR and deletions in 14q are found
20 %, 23 %, 3,3 %,13,3 %, respectively. No statistical significance were found between
gene alterations tumour grades and tumour localizations.
In conclusion 1p/14q deletions can be used as a marker for tumour grading an
progression in meningiomas.

Sex chromosome abnormalities are the most common chromosomal abnormalities detected on prenatal diagnosis and at birth with an estimated incidence of 1 in 448 newborns. The clinical severity of sex chromosome anomalies vary according to related aneuploidies and mosaic status and it is less than that associated with autosomal aneuploidy.
Disorders of sex development (DSDs) are congenital conditions in which development of the chromosomal, gonadal, or anatomic sex is atypical and may affect up to 1:1000 individuals in the population. The Genital phenotype has a very wide spectrum and in many cases the genotype-phenotype association is weak and it is difficult to distinguish pathologies with clear limits. In this groups Phenotypic findings are analyzed in relation to cytogenetic and molecular cytogenetics in order to determine etiology. In our study different clinical Characteristics of sexual development were investigated in order to evaluate the cytogenetic and molecular cytogenetic data in patient's peripheral blood, in Eskisehir Osmangazi University Faculty of Medicine Medical Genetics Department. Chromosome aberration frequency was evaluated in 469 cases that were referred with various clinical suspicions of sex chromosomal abnormalities, including Klinefelter Syndrome, Hypogonadotropic Hypogonadism, Ambigus Genitalia, Azoospermia, Turner Syndrome, Primary Amenorrhea and Primary İnfertility.
Karyotype results of the 469 patients revealed %87 with normal chromosome composition (n=408) and %13 with chromosomal abnormalities (n=61). Autosomal abnormalities were detected in 8 cases with a frequency of %13.11, whereas sex chromosome anomalies were identified in 53 cases with a frequency of 86.89%. In autosomal abnormalities the most commonly detected abnormalities were translocations and in sex chromosome abnormalities it was XXY. In sex chromosome abnormalities, the most commonly detected abnormality was in cases with Klinefelter Syndrome (50%). Followed by Hypogonadotropic Hypogonadism (27.27%), Ambiguous Genitale (21.43%), Azoospermia (14.2%), Turner Syndrome (12.33%), Primary Amenorrhea (7.4%) and Primary İnfertility (5.13).
In conclusion, retrospective evaluation of cytogenetic and molecular cytogenetic analysis of the referral cases over 3 years contributes to formation of regional data.

Toplam 61 anomalili olgunun %13.11'i (n=8) otozomal anomaliler, %86.89'u (n=53) cinsiyet kromozom anomalilerinden oluşmaktadır. Karyotipleme sonucunda, otozomal anomalilerde en çok translokasyon tipi anomaliler (n=5), cinsiyet kromozom anomalilerinde ise en çok 47,XXY (n=33) kromozom kuruluşu tespit edilmiştir. Periferik kandan elde edilen karyotip analizinde en çok Klinefelter Sendromu olgularında anomali tespit edilmiştir (%50). Takiben Hipogonadotropik Hipogonadism (%27.27), Ambigus Genitalya (%21.43), Azoospermi (%14.2), Turner Sendromu (%12.33), Primer Amenore (%7.4) ve Primer İnfertilite (%5.13) olgularında kromozomal anomaliler gözlenmiştir.

Serebral palsi gelişmekte olan beyinde progresif olmayan bir lezyon sonucu gelişen ama yıllar içinde progresyonu değişebilen, aktivite kısıtlılığına yol açan, kalıcı motor işlev, postür ve hareket bozukluğudur. Bu motor bozukluğa duyusal, bilişsel, davranışsal sorunlar, konvülsiyon ve sekonder kas iskelet sorunları eşlik edebilir. Son yıllarda serebral palsi ile ilgili çok sayıda genetik araştırma yapılmıştır. Çalışmamızda serebral palsi ile ilgili olduğu düşünülen Apolipoprotein E ve İnterlökin-6 genlerindeki polimorfizmleri araştırdık. Çalışmamıza toplam 78 serebral palsili olgu ve 60 kontrol dahil edildi. Tüm olgular hazırlanan değerlendirme formu doğrultusunda değerlendirildi ve genetik analizler gerçekleştirildi. Olguların klinik özellikleri ile polimorfizmler arasındaki olası ilişkiler tartışıldı. Çalışmamızda Apolipoprotein E polimorfizminin serebral palsi ile ilişkili olabileceği sonucuna ulaştık.
Cerebral palsy (CP) is a neurological disorder affecting movement and posture that develops as a complication of a non-progressive brain lesion which although can progress with time. The motor disfunction of CP may be accompanied with sensory, cognitive, behavioral problems, convulsion, secondary muscle and bone diseases. Recently there has been an interest in genetic researches about CP. This research is about the apolipoprotein E genotype and IL-6 polymorphism that has been thought to be related with CP. 78 patients with CP and 60 healthy individuals were analysed in the research. The control group and the patients were evaluated by a form and then went through genetic analysis. The clinical presentations and the polymorphisms were comparisoned and discussed. The research concluded that CP could be related with apolipoprotein E polymorphism.

Frontotemporal lobar dejenerasyon (FTLD); klinik, patolojik ve genetik
olarak heterojen progresif beyin hastalıkları grubunu tanımlamaktadır. Familyal
ALS (Amyotrofik lateral skleroz) ve FTD (Frontotemporal Demans)’nin major
sebebi olarak C9orf72 geninin kodlamayan bölgesindeki patojenik >30
heksanükleotid (GGGGCC) tekrar artışları tanımlanmıştır.
Çalışmamızda; FTLD spektrumunda C9orf72 GGGGCC tekrar artışlarının
Türk olgularda frekansının belirlenmesi ve bu tekrar artışlarının fenotip üzerindeki
etkilerinin saptanması amaçlanmıştır. FTLD spektrumundan tanı almış ve MAPT,
PGRN, CHMP2B, VCP, TARDBP, FUS genlerinde herhangi patojenik bir mutasyon
saptanmamış 100 olgu ile nörolojik/psikiyatrik olarak patolojik bir bulgusu
olmayan yaşla uyumlu 100 kontrol bireyinde bu heksanükleotid tekrar sayıları RPPCR
(repeat primed PCR) ve sizing-PCR yöntemleri kullanılarak belirlenmiştir.
Olgularımızın %67’si sporadik, %33’ü ailesel olarak sınıflanmış ve sporadik
olguların hiçbirinde patojenik tekrar artışı saptanmamıştır. Ancak ailesel
olgulardan bir olguda (1/33) patojenik aralıkta (>30) tekrar saptanmıştır. Olgu ve
kontroller arasında tekrar sayıları istatistiksel olarak anlamlı bulunmuştur.
Olgularımızın %4’ünde orta derece (20-30) tekrar saptanmıştır. Psikotik belirtilere
sahip hastalarda orta/patojenik tekrarların baskınlığı görülmüştür.
Çalışmamız Türkiye FTLD spektrumu için C9orf72 GGGGCC tekrar
artışlarının frekansının ve klinik üzerindeki etkisinin değerlendirilmesine yönelik
bilgimiz dahilindeki ilk çalışmadır. Çalışmamız sonucunda patojenik tekrar
artışlarının Türkiye FTLD olgularında çok yaygın görülmediği ancak orta derece
tekrarların FTLD için artmış bir risk faktörü olabileceği ya da bir modifiye edici gen
olarak görev yapabileceği düşünülmektedir. Bunun yanı sıra psikotik
semptomlarla bu orta/patojenik tekrar artışlarının korelasyonunun prognostik
açıdan önemli olabileceği düşüncesindeyiz. FTLD tanılı bir olguda sizing-PCR
yöntemiyle belirlemeyediğimiz patojenik tekrar sayısı, RP-PCR yöntemi
kullanılarak tespit edilmiştir. Bu nedenle iki yöntemin birlikte kullanılması tekrar
sayısının belirlenmesinde daha etkindir.
Sonuç olarak, Türk FTLD hastalarında C9orf72 GGGGCC ekspansiyon
frekansı için yeni veriler elde edilmiştir. Ancak, C9orf72 ekspansiyon frekansının
kesin olarak saptanması için verilerimiz Türk popülasyonundaki farklı etnik grup
FTLD hastalarında daha ileri çalışmalarla desteklenmesi gerekmektedir. Orta
derece tekrar içeren bireylerde yapılacak metilasyon, ekspresyon ve postmortem
çalışmalar C9orf72 geninde bu tekrarların patojenik etkisi hakkında bilgi verici
olacaktır. Bunun yanı sıra bu artışlar etrafındaki indellerin belirlenmesi ve ortak
PCR yöntemlerinin kullanılması C9orf72 popülasyon frekansının gerçek oranını
belirlemede daha etkili olacaktır.
Frontotemporal lobar degeneration (FTLD); describes a group of
progressive brain disorders that are clinically, pathologically, and genetically
heterogeneous. Pathogenic >30 repeats of a noncoding GGGGCC hexanucleotide
repeats in the C9orf72 gene have been defined as a major cause of both familial
FTLD and amyotrophic lateral sclerosis (ALS).
In our study; it was aimed to determine the prevalence of C9orf72 GGGGCC
repeat expansion in the Turkish cases with FTLD spectrum and to determine the
effects on the phenotype. The repeats in C9orf72 gene were analysed by the
repeat- primed PCR (RP-PCR) and sizing-PCR techniques in the FTLD-diagnosed
cases who had no mutations for FTLD-related genes (MAPT, PGRN, CHMP2B, VCP,
TARDBP, FUS) that had been previously analysed by the NGS technique. For
control group same age and healthy individuals without pathologic
neurological/psychiatric findings were analysed by the same techniques.
Of our cases, 67% were sporadic, 33% were classified as familial. No
pathogenic expansion was detected in any of the sporadic cases. However, the
pathogenic range expansion (>30) was found in one of the familial cases (1/33).
The allele length difference between the cases and controls was statistically
significant. An intermediate (20-30) repeat was detected in 4% of our cases. The
dominancy of intermediate/pathogenic repeats was seen in the cases with
psychotic symptoms.
This study is the first examination of C9orf72 GGGGCC repeat expansions
in the Turkish cases with FTLD spectrum. As a result of our study, it is thought
that C9orf72 pathogenic repeat expansion is not common in Turkish FTLD cases,
but intermediate repeat may be an increased risk factor for FTLD or may act as a
modifying gene. In addition, we believe that the correlation of these
intermediate/pathogenic repeats with psychotic symptoms may prognostically
important. In one case with FTLD, pathogenic repeats were not identified by
sizing-PCR whereas the pathogenic expansion was revealed by the RP-PCR.
Therefore, the combination of the both methods is more effective in determining
hexanucleotide expansions.
In conclusion, we presented novel data of the frequency of C9orf72
GGGGCC expansions in Turkish FTLD patients. However, our study data should be
supported by further studies in different ethnic groups for detection of C9orf72
expansion frequency in FTLD patients. Methylation, expression, and postmortem
studies should be conducted in cases who have intermediate repeat to gain insight
into the possible pathogenic effect for C9orf72 gene. Identification of the indels
around GGGGCC repeats and revealing the repeat size by same techniques would
be more effective in determining the true ratio of C9orf72 population frequency.

Kolorektal kanser dünyada kanser orjinli ölümlerin en önemli
nedenlerinden biridir. Metastazların, kanser nedenli ölümlerin büyük bölümünden
sorumlu olmasına ragmen kolorektal kanser metastazların moleküler yapıları
henüz çok az çalısılmıstır. Bu çalısma primer tümörler ile metastazlardaki
genomik anomalileri belirlemek ve böylece metastatik yayılıma yatkınlıga neden
olabilecek genetik olayları ortaya koyabilecek primer tümörler ile metastatik
tümörler arasındaki genetik farklılıklar saptanabilecektir. Çalısmada, 25 primer
tümör, 4 karaciger ve bir over metastazı ile 8 lenf nodu tümörü olmak üzere
toplam 38 tümör örnegi yüksek rezolusyonlu-karsılastırmalı genomik
hibridizasyon (YR-KGH) teknigi ile analiz edilmistir.
Bulgular, genomik kopya aberasyon frekansının dokudaki hücresel
farklılasma azaldıkça arttıgını göstermistir. Kromozom 8q, 13q, 20q artısları ile
kromozom 8p, 17p ve 18q delesyonları sıklıkla gözlenen anomalilerdi. Kromozom
2p, 7p12, ve 7p21 artısları ilk olarak bu çalısmada primer tümörlerde gözlenen
anomalilerdi. Kromozom 4q, 10q, 14q ve 20p artısları sadece primer tümörlerde
gözlenen anomalilerdi. Diger taraftan genomik kopya aberasyon sıklıgının
karaciger metastazlarında daha yüksek oldugu saptandı. Kromozom 9q ve 12p
artısları sadece karaciger metastazlarında gözlenen anomaliler iken kromozom
10p artısı sadece lenf nodu tümörlerinde gözlendi.
Tümörlerde yeni aday kromozomal bölgelerin saptanması ve daha önce
rapor edilen anomalilerin çalısmamızda daha yüksek sıklıkta gözlenmesinin
kullanılan teknikten kaynaklandıgı düsüncesindeyiz. Bilgilerimize göre bu çalısma
kolorektal tümörler ile metastazların YR-KGH teknigi ile incelendigi ilk
çalısmadır. Çalısma bulguları, tümörün yayılımındaki farklılıgın metastatik
fenotipe ait özelliklerden sorumlu spesifik degisimlerin gözlendigi rastgele
olmayan kromozom anomalilerinin birikimden kaynaklanabilecegini gösterir
niteliktedir. Ancak çalısma populasyonunun genisletilmesi ve ileri analizlerde elde
edilen verilerin hastalık prognozu ile baglantısının kurulmasının klinikte büyük
faydalar saglayacagı düsüncesindeyiz.
Colorectal cancer is one of the most common causes of cancer-related
death in the world. Although metastasis is responsible for most cancer deaths,
molecular basis of colorectal cancer metastasis remains poorly studied. This study
was undertaken to identify genomic imbalances specific to primary tumors and
metastatic tissues and so assess genetic differences between primary and metastatic
tumors that might identify genetic events predisposing to metastatic dissemination. In
total, 38 tumor specimens, including 25 primary tumors, 4 liver an done over
metastasis and 8 lymph node tumors, were analyzed by the High Resolution-
Comparative Genomic Hybridization (HR-CGH) technique.
The results showed that the frequency of genomic copy aberrations were
increasing in the tumors with poor cellular differentiation. Overrepresentations were
detected most frequently at 8q, 13q,20q and deletions were prominent at 8p, 17p and
18q. Chromosome 2p, 7p12 and 7p21gains were newly diagnosed aberrations in the
primary tumors. Gains on chromosomes 4q, 10q, 14q ve 20p were only seen in the
primary tumors. On the other hand, the frequency of genomic copy aberrations were
higher in the liver metastasis than the primary tumors. The gains on 9q and 12p were
specific aberrations in liver metastasis whereas 10p gains were seen in only lymph
node tumors.
We concluded that detection of new putative chromosomal regions in the
tumors and higher frequencies in previously reported non-random genomic copy
aberrations resulted from the used technique, HR-CGH whose resolution ability (>3-
4 Mb) is higher than the resolution capacity of KGH (>5-10 Mb). Depending on our
knowledge, this is the first study for HR-CGH analysis in colorectal tumors and
metastasis. The analysis suggested that the different pathways of tumor dissemination
are reflected by a nanrandom accumulation of chromosomal alterations with specific
changes being responsible for the different characteristics of the metastatic
phenotype. However, further analysis in larger populations are necessary and
combinations of these evidences with disease prognosis might be very helpful in
clinics.

Bu çalısmada, Türk populasyonunda VDR geni Bsm I ve Fok I
polimorfizmleri ile prostat kanseri arasındaki iliskiyi arastırmak amaçlanmıstır.
Prostat kanserli 75 hasta ve benign prostat hiperplazili 112 kontrolün
periferik kanından elde edilen DNA’larda Bsm I ve Fok I lokusları PCR ile çogaltılmıs,
ardından PCR ürünleri Bsm I ve Fok I restriksiyon enzimleri ile kesilmis ve agaroz jel
elektroforezi ile analiz edilmistir.
Yapılan çalısma sonucunda, VDR Fok I genotip frekansları hastalarda
%48.0 FF, %41.3 Ff ve % 10.7 ff; kontrollerde % 41.1 FF, % 48.2 Ff ve %10.7 ff olarak
saptanmıstır. F allel frekansı hastalarda % 89.3, kontrollerde ise %89.2 olarak
bulunmustur. f allel frekansı hastalarda %52.0, kontrollerde %58.9 olarak saptanmıstır.
VDR Bsm I frekansları hastalarda % 22.6 BB, % 41.3 Bb, % 36.0 bb;
kontrollerde %33.9 BB, % 42 Bb ve % 24.1 bb olarak saptanmıstır. B allel frekansı
hastalarda %64.0, kontrollerde % 75.9 olarak bulunmustur. b allel frekansı hastalarda %
77.3, kontrollerde % 66.1 olarak saptanmıstır.
Sonuç olarak, Türk populasyonunda Fok I ve Bsm I genotip dagılımlarının
prostat kanseri hastaları ve kontrollerde istatistiksel olarak önemli bir farklılık
göstermedigi (P> 0.05), dolayısıyla bu polimorfizmlerin prostat kanseri tanısında
genetik markır olarak kullanılmaya uygun olmadıkları görülmüstür. Hastaların
histopatolojik bulgulara göre gruplandırılması da sonucu degistirmemistir.
In this study, we aimed to investigate the association between VDR gene
Bsm I and Fok I polymorphisms with prostate cancer in Turkish population.
In the DNA’s extracted from pheripheral blood of 75 prostate cancer cases
and 112 benign prostatic hyperplasia controls, Bsm I and Fok I locuses were amplified
by PCR. Subsequently, PCR products have been digested by Bsm I and Fok I restriction
enzymes and results were analysed by agarose gel electrophoresis.
As result, VDR gene Fok I genotype frequencies were 48.0%, FF, 41.3 %,
Ff, 10.7 % ff in the cases; 41.1 %, FF; 48.2 %, Ff; and 10.7 %, ff in the controls. The
frequencies of F allels in the cases were 89.3 %, and were 89.2 % in the controls. The
frequencies of f allels were 52.0 % in the cases and were 58.9 % in the controls.
VDR gene Bsm I genotype frequencies were 22.6 %, BB, 41.3, Bb, 36.0
% bb in the cases; 33.9 %, BB; 42.0 %, Bb; and 24.1 %, bb in the controls. The
frequencies of B allels in the cases were 64.0 %, and were 75.9 % in the controls. The
frequencies of b allels were 77.3 % in the cases and were 66.1 % in the controls.
In conclusion, Fok I and Bsm I genotype distribution between prostate
cancer cases and controls were not significantly different (P>0.05) in Turkish
population. So, according to this study, we conclude that these polymorphisms are not
usefull as markers of prostate cancer. Stratification of cases and according to
pathological findings did not changed the results.

This study was planned to determinethe importance of convantional cytogenetic and molecular cytogenetic techniques in the prognosis of the cases with myelodysplastic syndrome (MDS). Totally 30 cases with myelodysplastic syndrome including 18 RA, 7 RAEB, 2 CMML, 2 RAEB-t and 1 RARS were analysed by the conventional cytogenetic and molecular cytogenetic (Fluorescent In Situ Hybridization) techniques. All the cases were diagnosed as primary MDS. Bone marrow samples of the cases were karyotyped by using the GTG banded metaphases and the FISH analyses were performed.
Karyotype analysis were failed in 8 of 30 cases (26.67%). Normal karyotypes were revealed in 13 cases (59.06%) whereas single chromosome abnormality was seen in 6 (27.2%), double chromosome abnormalities were detected in 2 (9.09%) cases. Complex karyotype was diagnosed in one case (4.55%).
In the FISH analyses, deletions of 5q31 in 4 cases (13%), 7q31 in 2 cases (6%), 7q31 and CEP7 in one case (3%), MLL gene region in 3 cases (10%), chromosome 20q in 3 cases (10%) were seen. Trisomy 8 was detected in two cases (6%) with additional chromosome abnormalities. We concluded that, FISH technique should be used in the diagnosis of genomic aberrations in the MDS cases to determine the prognosis of the disease. Detection of cryptic deletions and/or rearrangements by the FISH analyses allows to perform correct prognostic classification in the MDS cases.
Bu çalışma; myelodisplastik sendrom ( MDS ) tanısı alan olgularda hastalığın prognozu ile ilgili değerlendirmede konvansiyonel sitogenetik ve moleküler sitogenetik yöntemlerin önemini belirlemek amacıyla planlanmıştır. Bu amaç doğrultusunda MDS tanısı alan toplam 30 olgu klasik sitogenetik ve moleküler sitogenetik ( Floresan İn Situ Hibridizasyon (FISH)) analiz ile değerlendirilmiştir. Çalışmaya alınan olguların 18 tanesi RA, 1 tanesi RARS, 7 tanesi RAEB, 2 tanesi KMML, 2 tanesi RAEB-t idi. Olguların tümü primer MDS idi. Bütün olgulara GTG bantlama ile kromozom analizi ve FISH analizi yapılmıştır.
30 MDS lu olgunun 8 tanesinde (%26.67) sitogenetik çalışma için kromozom elde edilememiştir. 13 olguda (%59.06) normal karyotip izlenirken, 6 olguda (%27.2) tek kromozomal düzensizlik, 2 olguda (%9.09) çift kromozomal düzensizlik, 1 olguda (%4.55) kompleks kromozomal düzensizlik izlenmiştir.
Yapılan FISH çalışmalarında; olguların %13 ünde (4 olguda) 5q31 bölgesinde delesyon, %6 sında (2 olguda) 7q31 bölgesinde delesyon, %3 ünde (1 olguda) 7q31 ve CEP 7 bölgesinde delesyon (monozomi 7), %6 sında (2 olguda) trizomi 8, %10 unda (3 olguda) MLL gen bölgesinde delesyon ve %10 unda (3 olguda) 20q delesyonu gözlenmiştir.
Bu bulgular sonucunda; MDS li hastalarda sitogenetik analiz yanında, belli prognostik faktörler için FISH analizinin gerekliliğini ortaya koymuştur ve kriptik delesyon ya da yeniden düzenlenmelerin belirlenmesinde FISH analizinin kullanılması MDS hastalarında doğru prognastik sınıflamanın yapılmasını olanaklı kılmaktadır.

Çalışmamızda, Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Hastanesi Üroloji Polikliniğinde rektal muayene ve PSA (prostat spesifik antijeni) ölçümleri yapılmış, 71 prostat kanseri ve 119 kontrol grubu bireylerinden elde edilen DNA kullanılmıştır.
Genomik DNA lar Qiagen DNA ekstraksiyon kiti kullanılarak periferik kandan izole edildi.Daha sonra insülin geni 3’UTR bölgesinde yer alan INS+1127 Pst I polimorfizminin genotiplerini saptamak amacıyla, PCR yöntemiyle amplifiye edilip, PCR ürünleri Pst I restriksiyon enzimi kullanılarak kesilmiştir. Kesim ürünleri jel elektroforezine tabi tutularak , jelde görülen bantların büyüklüğüne göre genotipler tespit edilmiştir.
Çalışma sonucunda ; INS geni Pst1 polimorfizmi genotipleri prostat kanserli hastalarda (n:71) %67,6 CC, %28,2 CT, %4,2 TT , kontrol grubunda (n:119) %78,2 CC, %21 CT, %0,8 TT olarak belirlenmiştir.Veriler SPSS 11,0 paket programıyla istatiksel olarak analiz edilmiş , genotip frekanslarının karşılaştırılması X ² yöntemi kullanılarak yapılmıştır. Sonuçta hasta ve kontroller arasındaki genotip dağılımında anlamlı bir farklılık bulunamamıştır.( P= 0,078)
Prostat kanseri olgularında, Gleason skorları, genotip dağılımlarıyla Anova testi ile karşılaştırılmış; anlamlı bir farklılık bulunamamıştır (p=0,685).
The aim of this study, to determine the incidance of INS+1127 Pst I polymorphism, located in the 3’ UTR of insulin gene between prostate cancer cases and controls and to clarify, if these polymorphisms have any role in the etiology of prostate cancer.
This study, was done with DNA’s of 71 prostate cancer cases and 119 controls, who had been examined by digital rectal axamination and PSA measurement in the Urology polyclinic of Osmangazi University Faculty of Medicine.
By the help of Qiagen DNA extraction kit , the genomic DNA’s were isolated from the peripheral blood.The INS+1127 Pst I polymorphism whic lies in the 3’UTR area , were amplified with PCR method to determine their position.PCR product were digested by using restriction enzyme.After the digestion, product resolved gel electrophoresis.The genotypes were determined according to their band largeness seen in the gel.
At the end of study ;INS +1127 Pst I polymorphism genotypes out to be % 67.2 CC, 28.2% CT, 4.2%TT among prostatic cancer (n:71) individuals and 78.2% CC, 21% CT, 0.8% TT among control group individuals (n:119).
Data were analyzed using SPSS 11,0 statistical programme. Genotype frequency, comparison was made by using X² method. As a result, there was no significant difference between prostatic cancer individuals and control individuals ,in the genotype distribution (p= 0,132).
Gleason scores of prostatic cancer individuals, were compared with the genotype distributions, but there was no significant difference as well(p=0,685).

Akci er kanseri, tüm dünyada mortalitesi en yüksek kanser türüdür ve
kardiyovasküler hastalıklardan sonra ölüm nedenleri arasında ilk sıralarda yer almaktadır.
Çalı manın amacı Türkiye’de yeni bir yöntem olan Multiplex Ligation-dependent
Probe Amplification (MLPA) tekni i ile Akci er kanserlerine spesifik dizayn edilmi olan
Salsa MLPA Probemix 126/127 kitleri kullanılarak, akci er kanserleri ile ili kilendirilmi
64 farklı gen bölgesindeki delesyon ve / veya amplifikasyonların incelenerek ortaya
konmasıydı.
Çalı maya stanbul Yedikule Gö üs Hastalıkları Hastanesinde, histopatolojik olarak
incelenerek “ akci er kanseri” tanısı almı 100 olgu dahil edildi. Her hastaya ait parafine
gömülü doku örneklerinden, kanser dokusu içeren 5 mikronluk kesitlerden elde edilen
DNA örnekleri çalı ma grubu olarak, aynı hastaya ait kanser dokusu içermeyen kesitlerden
elde edilen DNA örnekleri de kontrol grubu olarak çalı maya alınmı tır.
Çalı mada, akci er kanserli tümöral dokularda en sık 2p, 3p, 13q, 17p ve 16p
delesyonları, yine tümöral dokulardan yapılan çalı malarda 17q, 8p ve 5q
amplifikasyonları görülmü tür. Tümöral alana kom u akci er dokularından lde edilen
DNA’ larda yapılan çalı malarda ise daha az sıklıkta olmak üzere tümöral alanlardakine
benzer bölgelerde delesyonlar görülmü tür. Ayrıca özellikle tümöral dokularda 5q, 8p,
9q, 10p, 11p. 11q, 12p, 14q, 17q ve 21q prob bölgelerinde de delesyon saptanmı tır.
Sonuç olarak MLPA yönteminin akci er kanserlerinin genomik de i imlerinin
taramasında kullanılabilecek ucuz, hızlı ve güvenilebilir sonuçlar verebilecek bir teknik
oldu u görülmü tür. Elde edilen bulguların literatürle uyumlu oldu u görülmü tür. MLPA
tekni i son zamanlarda geli tirilen en önemli moleküler tekniklerden birisidir ve kanser
ara tırmalarında da rahatlıkla kullanılabilecek bir yöntemdir.
Lung cancer has the leading mortality rate among all cancers and it is the second
most common cause of death following deaths from cardiovascular diseases.
This study was aimed to determine deleted and/or amplified regions of 64 different
genes regions previously associated with lung cancer by using Multiplex Ligationdependent
Probe Amplification (MLPA) technique and SALSA MLPA ProbeMix 126/127
kits which are designed specifically for lung cancer.
This study was performed by analyzing 100 archival samples which had been
previously examined histopathologically and diagnosed as “lung cancer” in stanbul
Yedikule Gö üs Hastalıkları Hastanesi. DNA extracts from 5 micron paraffin-embedded
tissue sections which contain cancerous tissues were used as sample group and 5 micron
paraffin-embedded tissue sections which contain well- differentiated normal tissues from
the same patients were used as control group in the study.
In the study, most frequently seen deletions in lung cancerous tissue were 2p, 3p,
13q, 17p, 16p and most frequently seen amplifications were17q, 8p ve 5q. In studies
performed in well-differentiated normal lung tissues, deletions in the same region as in
cancerous tissue were observed but they were in a lower frequencies. Deletions in 5q, 8p,
9q, 10p, 11p. 11q, 12p, 14q, 17q ve 21q probe regions were seen especially in cancerous
tissues.
As a conclusion, it was determined that MLPA is a technique which can give cheap,
fast and reliable results in screening lung cancers. The findings obtained in the study is
compatible with the literature. MLPA is one of the most important molecular techniques
which have been developed recently and it can be used in cancer screening easily and
reliably.

Akci er kanseri, tüm dünyada mortalitesi en yüksek kanser türüdür ve
kardiyovasküler hastalıklardan sonra ölüm nedenleri arasında 2. sırada yer almaktadır.
Metilasyon bir epigenetik mekanizma olup DNA düzeyinde herhangi bir mutasyon
olmamasına ra men ilgili genin eksprese olmasını engelleyen bir moleküler
mekanizmadır.
Çalı mada henüz çok yeni bir yöntem olan Multiplex Ligation-dependent Probe
Amplification (MLPA) tekni i ile akci er kanserleriyle daha önce ili kilendirilmi 24
ayrı tümör süpressör genin promoter bölgelerinin metilasyon paternlerinin SALSA MSMLPA
ME001 tumorsuppressor probemix kiti kullanılarak incelenmesi amaçlanmı tır.
Bu çalı maya stanbul Yedikule Gö üs Hastalıkları Hastanesinde, histopatolojik
olarak incelenerek “küçük hücreli olmayan akci er kanseri” tanısı almı 100 olgu dahil
edildi..Her hastaya ait doku örneklerinden kanser dokusu içeren 5 mikronluk kesitlerden
elde edilen DNA örnekleri çalı ma grubu olarak, aynı hastaya ait kanser dokusu
içermeyen kesitlerden elde edilen DNA örnekleri de kontrol grubu olarak çalı maya
alındı.
Çalı mada, akci er kanserli tümöral ve çevre akci er dokularında birbirlerine
yakın oranlarda en sık CDKN2B, BRCA1, CDH13 ve HIC1 prob bölgelerinde
metilasyon görüldü. Yine tümöral ve çevre dokularda PTEN, TIMP3, ATM, VHL,
CD44, CDKN1B, RASSF1, IGSF4 ve ESR1 prob bölgelerinde de metilasyon
saptandı. APC, CDKN2A, MLH1, RARB, CHFR ve GSTP1 prob bölgelerinde farklı
oranlarda, tümöral dokularda metilasyon görülürken, çevre akci er dokularında bu
prob bölgelerinde metilasyon saptanmadı.
Sonuç olarak MS MLPA yönteminin akci er kanserlerinin metilasyon profillerinin
taramasında kullanılabilecek, ucuz ve hızlı sonuç verebilen bir teknik oldu u
görülmü tür. MS MLPA yönteminin di er metilasyon tarama yöntemleriyle
kar ıla tırılarak bir an önce senstivite ve spesivitesinin belirlenmesi ve akci er
kanserinin erken tanısında rutin kullanıma geçirilmesi gerekti ini dü ünmekteyiz.

Gelismis ülkelerde kromozom anomalilerinin sebep oldugu sendromların prenatal
tanısı obstetrik takibin önemli bir parçasıdır. Anöploidi tanı testlerinde altın standart, klasik
kromozom analizidir (karyotipleme). Bununla birlikte Florasan In Situ Hibridizasyon ( FISH )
ve short tandem repeats (STRs) analizleri moleküler sitogenetik ve moleküler genetik testler
olarak spesifik anomalilerin tanısında rutin uygulamada kullanılmaktadır.
MLPA; farklı DNA dizilerinin tek bir reaksiyonda relatif kantitasyonuna imkan veren
yeni bir teknik olarak ortaya çıkmıstır. Retrospektif ve prospektif olarak yapılan çalısmamızda
anöploidi riski tasıyan 500 gebeden alınan amniyon sıvısında MLPA teknigi kullanılarak 13.,
18., 21., X ve Y kromozomlarının doz tayini yapılmıstır.
Çalısmamızda, riskli gebelerden alınarak Ana Bilim Dalı’mıza gönderilen amniyotik
sıvıdan 2 cc ayrılarak fetal hücrelerin DNA ları ekstrakte edilmistir. Fetal DNA daki 13., 18.,
21., X ve Y kromozomlarının dozajlarının belirlenmesi amacı ile bu kromozomlara spesifik
probların PCR ile amplifikasyonu yapılmıstır. Elde edilen PCR ürününde 310 Prizm Genetic
Analyzer cihazında Gene Mapper programı kullanılarak amplifiye olan probların miktarını
gösteren pikler elde edilmistir. Piklerin alanları Coffalyser programı ile analiz edilerek adı
geçen kromozomların doz tayini yapılmıstır.
MLPA teknigi ile elde edilen sonuçlar rutin yöntemlerle elde edilen sonuçlar ile
karsılastırılarak testin sensitivitesi ve spesifitesi belirlenmistir. MLPA tekniginin rutinde
kullanılabilirligi sınanmıs ve yeni yöntemin rutinde kullanılan yöntemlere göre avantaj ve
dezavantajları belirlenmistir.
Yapılan çalısma sonrasında örneklerin 480 tanesinde MLPA yöntemi ile sonuçlar tayin
edilebilmistir. Triploidi saptanan bir olgu (69,XXX) dısındaki, tüm olgu örneklerinde karyotip
sonuçları ile MLPA sonuçları uyumlu bulunmustur.
Prenatal diagnosis of chromosome syndromes is a long established part of obstetric
care in developed countries. Karyotyping has provided the gold standard as a diagnostic test.
Nevertheless, molecular tests, such as fluorescent in situ hiybridisation (FISH) and short
tandem repeats (STRs) analysis, are now in common practice for the diagnosis of spesific
abnormalities.
MLPA is a new technique, that provides relative quantification of different DNA
sequences by a single tube reaction. In our study, we determined retrospectively and
prospectively the dosages of 13., 18., 21., X and Y chromosomes from amniocentesis samples
of 500 pregnants by using MLPA technique.
The DNA is extracted from 2 ml amniotic fluid samples of pregnancies at increased
risk of aneuploidy that are referred to our department. The PCR reactions by using
chromosomes 13., 18., 21., X and Y specific probes performed to quantify the amounts of
these chromosomes in fetal DNAs. The amount of peaks related with the amplified probes
analysed by using Gene Mapper Programme in 310 ABI Prism Genetic Analyser System and
then each peak area analysed by the Coffalyser programme to determine doses of
chromosomes 13., 18., 21., X and Y.
By comparing the MLPA-detected results and the findings from conventional
cytogenetic analyses, we determined the specifity and sensitivity of the MLPA technique and
the usage of the MLPA technique as a routine diagnostic method for aneuploidy detection.
There was 480 conclusive MLPA tests. Results were concordant with those of
karyotyping with the exception of the 69, XXX karyotyped sample.

Bu çalısmada, Türk populasyonunda VDR geni Bsm I ve Fok I
polimorfizmleri ile prostat kanseri arasındaki iliskiyi arastırmak amaçlanmıstır.
Prostat kanserli 75 hasta ve benign prostat hiperplazili 112 kontrolün
periferik kanından elde edilen DNA’larda Bsm I ve Fok I lokusları PCR ile çogaltılmıs,
ardından PCR ürünleri Bsm I ve Fok I restriksiyon enzimleri ile kesilmis ve agaroz jel
elektroforezi ile analiz edilmistir.
Yapılan çalısma sonucunda, VDR Fok I genotip frekansları hastalarda
%48.0 FF, %41.3 Ff ve % 10.7 ff; kontrollerde % 41.1 FF, % 48.2 Ff ve %10.7 ff olarak
saptanmıstır. F allel frekansı hastalarda % 89.3, kontrollerde ise %89.2 olarak
bulunmustur. f allel frekansı hastalarda %52.0, kontrollerde %58.9 olarak saptanmıstır.
VDR Bsm I frekansları hastalarda % 22.6 BB, % 41.3 Bb, % 36.0 bb;
kontrollerde %33.9 BB, % 42 Bb ve % 24.1 bb olarak saptanmıstır. B allel frekansı
hastalarda %64.0, kontrollerde % 75.9 olarak bulunmustur. b allel frekansı hastalarda %
77.3, kontrollerde % 66.1 olarak saptanmıstır.
Sonuç olarak, Türk populasyonunda Fok I ve Bsm I genotip dagılımlarının
prostat kanseri hastaları ve kontrollerde istatistiksel olarak önemli bir farklılık
göstermedigi (P> 0.05), dolayısıyla bu polimorfizmlerin prostat kanseri tanısında
genetik markır olarak kullanılmaya uygun olmadıkları görülmüstür. Hastaların
histopatolojik bulgulara göre gruplandırılması da sonucu degistirmemistir.

Kolorektal kanser dünyada kanser orjinli ölümlerin en önemli
nedenlerinden biridir. Metastazların, kanser nedenli ölümlerin büyük bölümünden
sorumlu olmasına ragmen kolorektal kanser metastazların moleküler yapıları
henüz çok az çalısılmıstır. Bu çalısma primer tümörler ile metastazlardaki
genomik anomalileri belirlemek ve böylece metastatik yayılıma yatkınlıga neden
olabilecek genetik olayları ortaya koyabilecek primer tümörler ile metastatik
tümörler arasındaki genetik farklılıklar saptanabilecektir. Çalısmada, 25 primer
tümör, 4 karaciger ve bir over metastazı ile 8 lenf nodu tümörü olmak üzere
toplam 38 tümör örnegi yüksek rezolusyonlu-karsılastırmalı genomik
hibridizasyon (YR-KGH) teknigi ile analiz edilmistir.
Bulgular, genomik kopya aberasyon frekansının dokudaki hücresel
farklılasma azaldıkça arttıgını göstermistir. Kromozom 8q, 13q, 20q artısları ile
kromozom 8p, 17p ve 18q delesyonları sıklıkla gözlenen anomalilerdi. Kromozom
2p, 7p12, ve 7p21 artısları ilk olarak bu çalısmada primer tümörlerde gözlenen
anomalilerdi. Kromozom 4q, 10q, 14q ve 20p artısları sadece primer tümörlerde
gözlenen anomalilerdi. Diger taraftan genomik kopya aberasyon sıklıgının
karaciger metastazlarında daha yüksek oldugu saptandı. Kromozom 9q ve 12p
artısları sadece karaciger metastazlarında gözlenen anomaliler iken kromozom
10p artısı sadece lenf nodu tümörlerinde gözlendi.
Tümörlerde yeni aday kromozomal bölgelerin saptanması ve daha önce
rapor edilen anomalilerin çalısmamızda daha yüksek sıklıkta gözlenmesinin
kullanılan teknikten kaynaklandıgı düsüncesindeyiz. Bilgilerimize göre bu çalısma
kolorektal tümörler ile metastazların YR-KGH teknigi ile incelendigi ilk
çalısmadır. Çalısma bulguları, tümörün yayılımındaki farklılıgın metastatik
fenotipe ait özelliklerden sorumlu spesifik degisimlerin gözlendigi rastgele
olmayan kromozom anomalilerinin birikimden kaynaklanabilecegini gösterir
niteliktedir. Ancak çalısma populasyonunun genisletilmesi ve ileri analizlerde elde
edilen verilerin hastalık prognozu ile baglantısının kurulmasının klinikte büyük
faydalar saglayacagı düsüncesindeyiz.
Colorectal cancer is one of the most common causes of cancer-related
death in the world. Although metastasis is responsible for most cancer deaths, molecular
basis of colorectal cancer metastasis remains poorly studied. This study was undertaken
to identify genomic imbalances specific to primary tumors and metastatic tissues and so
assess genetic differences between primary and metastatic tumors that might identify
genetic events predisposing to metastatic dissemination. In total, 38 tumor specimens,
including 25 primary tumors, 4 liver an done over metastasis and 8 lymph node tumors,
were analyzed by the High Resolution- Comparative Genomic Hybridization (HRCGH) technique. In this method, DNAs from paraffin embedded tumor tissues (test
DNA) and from peripheral blood of a healthy man (control DNA) were amplified by
degenerated-oligonucleotide primed-polymerase chain reaction (DOP-PCR) and labeled
with spectrum gren and spectrum red, respectively. Then the labeled DNAs were
applied onto normal lymphocyte metaphase preperations in hybridization. The HRCGH profiles were analyzed and the genomic copy aberrations were compared in the
groups. The results showed that the frequency of genomic copy aberrations were
increasing in the tumors with poor cellular differentiation. Overrepresentations were
detected most frequently at 8q, 13q,20q and deletions were prominent at 8p, 17p and
18q. Chromosome 2p, 7p12 and 7p21gains were newly diagnosed aberrations in the
primary tumors. Gains on chromosomes 4q, 10q, 14q ve 20p were only seen in the
primary tumors. On the other hand, the frequency of genomic copy aberrations were
higher in the liver metastasis than the primary tumors. The gains on 9q and 12p were
specific aberrations in liver metastasis whereas 10p gains were seen in only lymph node
tumors.
We concluded that detection of new putative chromosomal regions in the
tumors and higher frequencies in previously reported non-random genomic copy
aberrations resulted from the used technique, HR-CGH whose resolution ability (>3-4
Mb) is higher than the resolution capacity of KGH (>5-10 Mb). Depending on our
knowledge, this is the first study for HR-CGH analysis in colorectal tumors and
metastasis. The analysis suggested that the different pathways of tumor dissemination
are reflected by a nanrandom accumulation of chromosomal alterations with specific
changes being responsible for the different characteristics of the metastatic phenotype.
However, further analysis in larger populations are necessary and combinations of
these evidences with disease prognosis might be very helpful in clinics

Amaç: Degisici Epitel Karsinomu (DEK) mesane kanserinin en yaygın
türüdür. Kromozom aberasyonları DEK nun gelismesinde ve ilerlemesinde
gereklidir. Bu çalısmada, mesane degisici epitel karsinomu (DEK) tanısında
noninvaziv yaklasımla interfaz FISH tekniginin potansiyel klinik uygulamasını
belirlemek ve kromozomal anöploidilerle tümörün derecesi ve evresi arasındaki
iliskiyi arastırmak amaçlanmıstır.
Materyal ve Metot: Üroloji bölümünden degisici epitel karsinoma tanısı
almıs 30 hasta ve saglıklı 15 kontrolden, bosaltılmıs idrar ve mesane yıkama ile
hücreler elde edilmistir. Bu hücrelerde 3, 7, 8, 11 ve 17 kromozomların
sentromerlerine özgü problar ve 9p21 lokus spesifik problar kullanılarak FISH
analizi yapılmıstır.
Bulgular: FISH analizinin DEK belirlemesinde sensitivitesi mesane yıkama
ve idrar örnekleri için sırasıyla % 96.7 ve % 92.6, spesifitesi ise % 100 olarak
bulunmustur. G1-G2 (Düsük) tümörlerin mesane yıkama ve idrar örneklerinde
görülen en sık kromozom anomalisi 9p21 delesyonu (% 66-% 75) dur. Kromozom
3, 7, 8, 11 ve 17 polizomisinin T1 G2-G3(Yüksek) ve T2 G3(Yüksek) karsinomlara
spesifik oldugu bulunmustur. G1-G2 (Düsük) ve G2-G3(Yüksek) tümörleri
arasında kromozom 3, 7, 8, 11 ve 17 polizomi frekansında istatistiksel olarak
anlamlı farklılık oldugu bulunmustur (P<0,01).
Sonuç: FISH analizi, mesane yıkama ve idrar örneklerinden elde edilen
üretelyal hücrelerde kromozomal aberasyonları belirleyerek, degisici epitel
karsinomu tanısında klinige katkı saglayabilecek potansiyele sahip noninvaziv bir
tekniktir. Kromozom polizomisi ile yüksek tümör derecesi ve evresi koraledir.

Objective: Transitional Cell Carcinoma (TCC) is the most common form of
bladder cancer. Chromosomal aberrations are involved in the development and
progression of TCC of bladder. In this study, we aimed to determine the potential
for the clinical application of interphase FISH technique with noninvaziv approach
in diagnosis of transitional cell carcinoma (TCC) and to investigate the relation
between chromosomal aneuploidies with tumor grades and stage.
Methods: The cells were obtained by bladder washing and voided urine from
30 patients who had a transitional cell carcinoma (TCC) of the bladder diagnosis
and 15 healthy controls in the Urology Department. In the samples, FISH analysis
was performed by using centromere specific probes for chromosome 3, 7, 8, 11, 17
and locus specific probe of 9p21.
Results: The sensitivities of FISH to detect TCC for bladder washing and
voided urine were found 96.7 % and 92.6 %, respectively, but its specificity was
found 100%. The most frequently seen chromosomal alterations G1-G2(Low)
tumors were deletion of 9p21 (66 % and 75 %) in bladder washing and voided
urine. Polysomy was specific to T1G2-G3(High) and T2G3(High) carcinomas. A
statistically significant difference was seen in the frequency of polysomy between
G1-G2(Low) and G2-G3(High) for chromosomes 3, 7, 8, 11 and 17 (P<0.01).
Conclusions: FISH assays have potential that can provide contribution to
clinical diagnosis of transitional cell carcinoma to detect chromosomal aberrations
present in urothelial cells obtained from voided urine and bladder washing.

Osteoporoz, kemik mineral yoğunluğunun (KMY) azalması ve kemik dokunun
mikromimari yapısının bozulması ile karakterize multifaktöriyel bir hastalıktır.
Kemik mineral yoğunluğunu etkileyen birçok çevresel faktör olmasına rağmen son
yıllarda yapılan çalışmalarda genetik yapının osteoporoz patogenezindeki etkisi
üzerinde durulmuştur. Kemik kütlesi üzerine etkisi olan birçok aday genin varlığı
rapor edilmiştir. Ancak bu genlerin kemik kütlesi üzerine olan etkisi ile birbirleri ve
çevresel etkenlerle olan etkileşimleri halen net değildir.
Bu çalışmada, 126 postmenopozal kadında (kemik mineral yoğunluğu
açısından 30 normal, 46 osteopenik ve 50 osteoporotik) lomber omurga ve femur
boynu KMY değerleri ile ERα geni PvuII, XbaI polimorfizmleri ve COL1A1 geni
Sp1 polimorfizmi arasındaki ilişki araştırılmıştır. Ayrıca çalışmamızda bu
polimorfizmlere ait genotip ve alel frekanslarının normal, osteopenik ve osteoporotik
olgulardaki dağılımı incelenmiştir.
Çalışma sonuçlarımıza göre ERα geni PvuII polimorfizmi, pp genotip frekansı
açısından normal olgular ile osteopenik ve osteoporotik olgular arasında anlamlı fark
bulundu. pp genotip frekansı, normal olgularda anlamlı düzeyde düşüktü. Yine ERα
geni PvuII polimorfizmine ait P ve p alel frekansları değerlendirildiğinde
osteoporotik olguların p alel frekansı normal olgulardan anlamlı düzeyde yüksekti.
PP genotipine sahip olguların lomber omurga ortalama KMY değeri, pp genotipli
olguların lomber omurga ortalama KMY değerinden anlamlı düzeyde yüksekti.
Diğer taraftan ERα geni XbaI polimorfizmi ile COL1A1 geni Sp1 polimorfizminde
yapılan değerlendirmelerde gruplar arasında KMY değerleri, genotip ve alel
frekansları açısından fark bulunmadı. Sonuç olarak, çalışma grubumuzdaki
postmenopozal kadınlarda lomber omurga KMY değeri üzerine ERα geni PvuII
polimorfizminin etkili olduğu gösterilmiştir.
Osteoporosis is a multifactorial disease characterized by a decrease in bone mineral
density (BMD) and micro-architectural deterioration of bone structure. Although there are
several environmental influences on BMD, a genetic contribution to the pathogenesis of
osteoroposis has recently been recognized. The existence of many candidate genes which
have effect on bone mass was reported. However, the effect of these genes' on bone mass, the
interaction among these genes and interaction among these genes and enviromental effects are
not presently clear.
In this study, the relationship among BMD values of lomber vertebra and femoral
neck, and ERα gene PvuII, XbaI polymorphisms and COL1A1 gene Sp1 polymorphism in 126
postmenopausal women (30 normal, 46 osteopenic and 50 osteoporotic in terms of bone
mineral density) was researched. Besides, the distribution of genotype and allele frequencies
belonging to these polymorphisms in normal, osteopenic and osteoporotic postmenopausal
women was also evaluated in this study.
According to our study results, significant difference was found in among normal
women, and osteopenic and osteoporotic women in terms of ERα gene PvuII polymorphism
pp genotype frequency. pp genotype frequency was significantly lower in normal women.
When P and p allele frequencies belonging to ERα gene PvuII polymorphism were evaluated,
it was observed that p allele frequency of osteoporotic women was significantly higher than
that of normal women. Lomber vertebra average BMD value of women with PP genotype was
significantly higher than that of with pp genotype. On the other hand, in the evaluations on
ERα gene XbaI polymorphism and COL1A1 gene Sp1 polymorphism, it was noted that there
was no difference in terms of BMD values, genotype and allele frequencies among groups. In
conclusion, it was designated that ERα gene PvuII polymorphism was effective on lomber
vertebra BMD value in postmenopausal women of our study group.

Meme kanseri dünyada kadınlar arasında en sık görülen kanser türüdür. Son
yıllarda mikroRNAların işlevlerinin ve kanserdeki rollerinin anlaşılmaya başlanması
hem kanserin moleküler patolojisinin anlaşılmasında hem de yeni moleküler hedefe
yönelik tedaviler geliştirilmesinde umut olmuştur.
Çalışmamızda 28 meme kanserli hastanın kan ve doku örnekleri ve 50 meme
kanseri açısından sağlıklı bireyin kan örneklerinde miR-34a ve miR-125a
mikroRNAlarının genomik dizileri dizileme yöntemi ile incelenmiştir. miR-34a’nın
genomik bölgesinde herhangi bir polimorfizm veya mutasyon saptanmamıştır. miR-
125a’nın transkribe olmayan bölgesinde bilinen bir polimorfizm kontrol ve hasta
örneklerinde belirlenmiştir. Bu polimorfizmin hastalara ait örneklerdeki genotip
dağılımı; %50,0 TT,%32,1 CT, %17,9 CC şeklindedir. Kontrol örneklerindeki dağılımı
ise % 56,0 TT,% 36,0 TC, %8,0 CC’ dir. Kontroller ve meme kanserli örnekler arasında
istatistiksel olarak anlamlı bir fark bulunmamıştır (p>0,05).
Polimorfizmin bulunduğu bölge, internet tabanlı bir programla transkripsiyon
faktörleri için bağlanma bölgesi taşıyıp taşımadığı yönünde incelenmiştir. Bu
polimorfizmin bulunduğu bölgeye TT genotipi için Gata-1 transkripsiyon faktörü
bağlanırken, CC genotipi için Sp-1 transkripsiyon faktörü bağlanmaktadır. Sp-1 ER
için koaktivatör görevi gördüğü için meme kanseri gelişiminde rol oynadığı
bilinmektedir.
Çalışmamızda meme kanserinde miRNA mutasyonlarının seyrek görüldüğü
sonucuna varılmış olsa da, net sonuca varmak için bu konuda daha çok çalışma
yapılması gerekmektedir. Çalışmamız miR-125a’nın meme kanserindeki önemini ortaya
koymuştur ve miR-125a’nın hücresel yolaklarla bağlantısının kurulması için fonksiyonel
analizler yapılması gerekliliğini vurgulamıştır.
Breast cancer is the type of cancer most frequently seen among women in the
world. Recently understanding the functions of microRNAs and their roles in cancer
progression has been providing expectations for understanding the molecular pathology
of breast cancer and developing new molecular targeted therapies.
In our study, genomic sequences of miR-34a and miR-125a were studied in tissue
and blood samples from 28 patients with breast cancer and 50 healthy controls with
respect to breast cancer by DNA sequencing. Any mutation or polymorphisms were not
detected in genomic sequence of miR-34a. A known polymorphism in non-transcribed
region of miR-125a was found in patients and control cases. The genotype frequencies of
this polymorphism in patient group are 50.0 % TT, 17.9 % CC, 32.1 % CT. The
frequencies in control group are 56.0% TT, 8.0 %CC, 36.0 % TC. The genotype
frequency difference between patient and control group is statically not significant
(p>0.05).
The region containing this polymorphism was analyzed whether it contains a
binding site for a transcription factor or not. While Gata-1 transcription factor binds
this region for TT genotype, Sp-1 transcription factor binds this region for CC genotype.
Because Sp-1 is a coactivator for ER, it is known that Sp-1 takes part in breast cancer
progression.
In our study, although it was concluded that miRNA mutations in breast cancer
is a rare event more studies are needed to perform to drive an exact conclusion. Our
study showed the importance of miR-125a in breast cancer and it emphasized the
necessity of performing functional analysis to associate miR-125a with cellular pathways
in breast cancer.

Demans kişinin bilişsel ve entellektüel işlevlerinin normal dışı azalmasıdır.
Demans sendromlarının en yaygın olanı Alzheimer hastalığıdır. Alzheimer
nörofibriler yumaklar ve amiloid plaklar ile karakterize, yıkıcı bir beyin hastalığıdır.
Alzheimer multifaktöryel kalıtım gösteren bir hastalık olup; genetik temeli oldukça
karmaşıktır. Birçok farklı gen ve polimorfizm çevresel faktörlerle etkileşerek demans
gelişimine sebep olabilmektedir. Geç başlangıçlı ve sporadik Alzheimer
patogenezinde etkili olduğu düşünülen en önemli genetik risk faktörü ApoE gen
polimorfizmidir. Ayrıca inflamasyon, Alzheimer hastalığına neden olan patojenik
kaskatların bir kısmı olabilir. Patolojik çalışmalar Alzheimer hastalarının beyninde
kronik inflamasyonun olduğunu ortaya koymuştur. Bu sebeple, bir proinflammator
sitokin olan interlökin-1 (IL-1) de bu hastalığın patogeneziyle ilişkilendirilmiştir.
Birçok çalışma, Alzheimer hastalığı ve IL-1α C889T alel polimorfizmi arasında
ilişki olduğunu göstermektedir.
Bu çalışmada, ApoE geni ε2, ε3, ε4 polimorfizmleri ve IL-1α geni C889T
polimorfizminin genotip ve alel frekansları çalışma grubumuzdaki 95 Alzheimer
hastası ve 60 demans öyküsü olmayan kontrol bireyde değerlendirilmiştir.
Çalışmamızda ε3/ε3 genotip frekansı kontrol grubunda, ε3/ε4, ε4/ε4 genotip
frekansları ise olgu grubunda anlamlı düzeyde yüksekti. ε3 alel frekansı kontrol
grubunda, ε2 ve ε4 alel frekanasları ise olgu grubunda anlamlı düzeyde yüksekti.
Kontrol ve olgu grupları arasında IL-1α geni C889T polimorfizmine ilişkin genotip
ve alel frekanslarının dağılımı açısından farklılık gözlenmedi.
Sonuç olarak, Alzheimer hastalığı ile Apo E ε4 aleli arasında kuvvetli bir ilişki
varken, Alzheimer hastalığı ile IL-1α C889T polimorfizmi arasında bir ilişki
olmadığı belirlendi.
Demantia is abnormal decrease of cognitive and intellectual processes. The most
commonly demantia syndrome is Alzheimer’s disease. Alzheimer’s disease is a destructive
brain disease which is categorized by neurofibrillar tangles and amyloid plaques. Alzheimer’s
disease is a multifactorial disease and its genetic basis is too complicated. Many different
genes and polymorphisms interacting with environmental factors may lead to development of
demantia. ApoE gene polymorphism is thought to be the most important genetic risk factor in
the patogenesis of late onset and sporadic Alzheimer’s disease. Inflammation may also be the
part of patogenic cascade that leads to the Alzheimer’s disease. Pathological studies have
exhibited that chronic inflammation exists in the brain of patients with Alzheimer’s disease.
Therefore, Interleukin 1 (IL-1), a proinflammatory cytokine, is found to be associated with the
patogenesis of Alzheimer’s disease. Many investigations show that there is an association
between IL-1α C889T allele polymorphism and Alzheimer’s disease.
In this study recent studies, genotype and allele frequencies of ε2,ε3,ε4 polymorphisms
of ApoE gene and C889T polymorphism of IL-1α gene were evaluated in 95 patients with
Alzheimer’s disease and 60 controls without demantia story. The results showed that ε3/ε3
and ε3/ε4 genotype frequencies significantly higher in the control and case groups,
respectively. Although statistically significant ε3 allele frequency was detected in the control
group, ε2 and ε4 allele frequencies were significantly higher among the cases with
Alzheimer’s. There was no difference between control and case group in point of the
distribution of genotypes and allele frequencies of C889T polymorphism of IL-1α gene.
In conclusion, there was a strong association between Alzheimer’s disease and ApoE ε4
allele, while was no relation was seen in with IL-1α C889T polymorphism.

Varfarin, tromboembolik olayların tedavisinde ve önlenmesinde tüm dünyada
geniş bir şekilde kullanılan oral antikoagülandır. Varfarine karşı cevap bireyler
arasında çok değişkenlik göstermekte ve bu ilaca bağlı ciddi kanama olayları
gerçekleşmektedir. Varfarin farmakogenetiği ile ilgili majör genler CYP2C9 ve
VKORC1 genleridir. Varfarin tedavisinde genetik ve çevresel faktörlerin bir arada
değerlendirilmesi tedavinin bireyselleştirilmesi ve daha güvenli bir şekilde
uygulanması açısından yardımcı olmaktadır.
Bursa Uludağ Üniversitesi Kardiyoloji Ana Bilimdalı’nda tedavi görmekte olan
234 varfarin kullanan olguda bireylerarası varfarin doz değişkenliğinin belirlenmesi ve
ayrıca Eskişehir Osmangazi Üniversitesi Tıbbi Genetik Ana Bilimdalı’nın belirlemiş
olduğu 200 sağlıklı bireyinde çalışmaya dâhil edilmesiyle, Türk populasyonu genotip
dağılımının belirlenmesi amacıyla CYP2C9 ve VKORC1 gen polimorfizmleri PCRRFLP
yöntemi kullanılarak incelenmiştir.
Çalışmamıza katılan 434 bireyde, VKORC1 geni -1639 G>A polimorfizmi
genotip dağılımları G/G için %27,6, G/A için %48,4 ve A/A için %24 olarak
bulunmuştur. CYP2C9 geni 430 C>T ve 1075 A>C polimorfizmlerine göre CYP2C9
genotip varyant frekansları, *1*1 frekansı %59,4, *1*2 frekansı %19,8, *1*3 frekansı
%13,8, *2*2 frekansı %3,7, *2*3 frekansı %2,8 ve *3*3 frekansı ise %0,5 olarak
saptanmıştır. Varfarin kullanan olgularda ortalama günlük varfarin doz miktarı ile
CYP2C9 ve VKORC1 genotipleri arasında istatistiksel olarak anlamlı bir ilişki
bulunmuştur. CYP2C9, VKORC1 ve yaş faktörlerinin bireylerarası varfarin doz
değişkenliği üzerine %29 etkisi olduğu istatistiksel olarak saptanmıştır.
Çalışmamız, Türk populasyonunda bireylerarası varfarin doz değişkenliğinin
belirlenmesi ve klinikte olguların varfarin dozaj tespitinde yardımcı genetik faktörler
olarak kullanılabilmesi açısından tedavide uygulanabilirliği olduğunu düşünmekteyiz.
Warfarin is a widely used oral anticoagulant in the world to prevent and treat
tromboembolic cases. Response to warfarin differ very much between individuals and
serious bleedings may occur due to this drug. Major genes about warfarin
pharmacogenetics are CYP2C9 and VKORC1. Evaluating genetic and environmental
factors together in treatment protocols assist in application of treatment more safely
and individualization of treatment.
234 patients using warfarin from Cardiology Department Bursa Uludağ
University were analyzed for CYPC2C9 and VKORC1 gene polymorphisms to
determine warfarin dose variations among patients and 200 healthy controls selected in
Medical Genetics Department in Eskişehir Osmangazi University were analyzed for
CYPC2C9 and VKORC1 gene polymorphisms to determine frequencies of the
genotypes in Turkish population PCR-RFLP technique was used in study.
For 434 cases included our study, frequencies of VKORC1 gene -1639 G>A
are, %27,6, %48,4, %24 for G/G, G/A, A/A genotypes respectively. For CYP2C9 gene
430 C>T and 1075 A>C polymorphisms CYP2C9 genotype variant frequencies are
%59,4, %19,8, %13,8, %3,7, %2,8, %0,5 for *1*1,*1*2,*1*3,*2*2,*2*3,*3*3 variants
respectively. A statistically significant relation was determined among average daily
warfarin dose and CYP2C9 and VKORC1 genotypes in cases using warfarin. It was
statically found that CYP2C9, VKORC1 and age factors has %29 effect on warfarin
dose variations among cases.
Our study emphasizes importance of genetic factors in determining warfarin
dose variations among cases in Turkish Population and to using these genetic factors to
help warfarin dose determination in clinical cases.

Parkinson Hastalığı ile ilişkilendirilmiş genlerin delesyon ve amplifikasyonları Multiplex Ligation-dependent Probe Amplification ( MLPA) yöntemiyle incelenmiştir.
Ailesel Parkinson Hastalığına sebep olan genlerde saptanan genomik değişimler sporadik Parkinson Hastalarında da görülmektedir. Literatürde Parkinson Hastalığından sorumlu olduğu bilinen SNCA, UCH-L1, DJ–1, Parkin, PINK1 ve LRRK2 genleri saptanmıştır.
Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Nöroloji Anabilim Dalı Hareket Bozukluğu Polikliniğine 2004–2008 yılları arasında Londra Beyin Bankası tanı kriterlerine göre sporadik Parkinson hastalığı tanısı almış 58 sporadik Parkinson hastasının kanlarından elde edilen DNA ile MLPA analizi yapılmıştır. Bu hastalarda Salsa MLPA P051 ve P052B kitleri ile çalışılmış, SNCA geninde 10 delesyon, 12 amplifikasyon, 14 heterozigot A30P, bir homozigot A30P nokta mutasyonu saptanmıştır. DJ–1 geninde 13 delesyon, dokuz amplifikasyon, Parkin geninde 16 delesyon, sekiz amplifikasyon, PINK1 geninde 14 delesyon, 21 amplifikasyon bulunmuştur. LRRK2 geninde dört delesyon, yedi amplifikasyon ve yedi G2019S homozigot nokta mutasyonu tesbit edilmiştir. UCH-L1 geninde ise herhangi bir delesyon veya amplifikasyon saptanmamıştır.
Literatürde DJ–1 ve PINK1 genlerinin amplifikasyonlarına rastlanmamış olmasına rağmen, MLPA analizi sonucumuza göre olgularımızda bu her iki gende de amplifikasyonlar tesbit edilmiştir.
Çalışmamız Türkiye’de sporadik Parkinson hastalarının MLPA analizi yöntemiyle incelendiği ilk araştırma olmuştur.

Akciğer kanserleri; artan tedavi olanaklarına rağmen tüm dünyada mortalitesi en yüksek olan kanser türüdür. Günümüzde aynı evre ve histopatolojik özelliklere sahip olan akciğer kanserlerinde benzer tedaviler verilmektedir. Ancak kemoterapiye yanıt, prognoz ve yaşam sürelerindeki farklılıklar halen açıklanamamaktadır ve bu konuda rutin olarak kullanılan testler henüz yetersizdir. Bu çalışmada yarı invaziv metodla alınan bronşial lavaj örneğinde; akciğer kanserine özgü genetik markerlerin özgüllük ve özgünlüğünün araştırılması hedeflenmiştir. Aynı zamanda EGFR, C-MYC, HER2’nin kemoterapiye yanıt, survi ve prognoz ile ilişkisi değerlendirilmiştir.
Bu çalışmada 100 akciğer kanserli olgunun 81’i Küçük Hücreli Dışı Akciğer Kanserli (KHDAK), 16’sı Küçük Hücreli Akciğer Kanserli (KHAK), 3’ü de metastatik akciğer kanseri olarak histopatolojik tanı almışlardır. Kontrol grubu olarak; akciğer kanseri olmayan 15 olgunun bronşial lavaj materyali FISH yöntemiyle analiz edilmiş ve normal olarak değerlendirilmiştir.
Ayrıca 100 olgudan 81’inin kesin histopatolojik tanıları 1. fiberoptik bronkoskopide (1.FOB) alınan biyopsi materyali analiziyle konulmuş; 19 olgunun kesin histopatolojik tanıları ise daha sonra yapılan 2.FOB, iğne biyopsisi, cerrahi yöntemlerle alınan biyopsi materyalleri analiziyle konulabilmiştir. 1.FOB’de kesin histopatolojik tanı konulamayan 19 olgudan 8’inde FISH pozitif bulgular saptanmıştır. Sadece üç genetik marker kullanılarak yapılan (1.FOB’de alınan) bronşial lavaj materyali analiziyle 100 olgudan 65’inde FISH pozitif sonuçlar elde edilmiştir. Kombine (histopatoloji ve FISH) olarak birlikte değerlendirildiğinde ise 89 olguda (% 89) anomali saptanmıştır. Bu çalışmada histopatolojinin sensitivitesi
% 81, spesifitesi % 100; FISH analiz yönteminin sensitivitesi % 65, spesifitesi % 100 olarak bulunmuştur.
FISH yöntemiyle analiz edilen 100 akciğer kanserli olguda; EGFR probu ile
% 46, C-MYC probu ile % 38, HER2 probu ile % 35 olguda FISH pozitif (+) sonuçlar saptanmıştır ve kemoterapiye yanıt, prognoz, survive ile ilişkileri değerlendirilmiştir. Verilerimiz, kullanılan üç genetik marker ile dahi, tanı ve tedaviye ilişkin bilgilere ulaşılabildiğini göstermiştir. Buna bağlı olarak bronkoskopi ile alınan bronşial lavaj örnekleri, farklı genetik markerlar kullanılarak analiz edildiğinde; erken tanı, prognoz değerlendirme ve hatta tedaviye yol gösterici olarak değerli bilgiler vereceği düşüncesindeyiz.
Despite of improving treatment protocols, lung cancers are still the most lethal types of cancers. Similar treatments are used in patients with the lung cancer at same stages and have histopathological features. However, differences in prognosis, chemotherapy response and survival time can not be explained yet and the tests used are not sufficient to answer these questions. The goal in this study was to prospectively aimed the sensitivity and specity of FISH detected genetic markers of lung cancer from semi non-invasive bronchial lavage samples of the patients undergoing bronchoscopy for suspected lung cancer. In the same time, the relations of FISH-detected EGFR, C-MYC, and HER2 gene copy number variations with the prognosis, chemotherapy response and survival time differences were evaluated.
In this study, analyses were performed on one hundred patients referred for bronchoscopy for suspicion of lung cancer. The histopathologic results of one hundred cases were as following: 81 NSCLC, 16 SCLC and three metastatic lung cancer. The samples from 15 cases without lung cancer suspicion but with bronchoscopic indication for differential diagnosis served as control samples, for bronchial lavage material, evaluated as normal.
During the first fiberoptic bronchoscopy (1.FOB), the histopathological diagnosis could be revealed in 81 cases of 100. The diagnosis of the remaining cases could be determined through the second FOB or needle biopsy or surgical applications. Of the 19 cases in which histopathological diagnosis could not be determined in the first FOB, eight of them showed cancer-related FISH positive results. Although only three FISH-detected genetic markers used, FISH positive results from the first FOB samples could be determined in 65 samples. In the bronchial lavage samples were obtained from first FOB, FISH positive results were obtained in 65 of 100 cases. The sensitivity and specity of histopathological analysis were 81 % and 100 % whereas they were 65% and 100% for FISH analysis, respectively. The combined histopathological and FISH analyses showed positive results in 89 cases (89%).
Of one hundred cases with lung cancer, 46% had EGFR, 38% had C-MYC and 35% had HER2 gene copy number abnormalities. The relations of these FISHdetected results with the prognosis, chemotherapy response and survive were evaluated. The data showed that even with three genetic markers relating to the diagnostic and prognostic clues could be reached. Therefore, the samples obtained from semi-invasive bronchial lavage technique might be informative in early diagnosis, prognosis determination, even in therapy determination if different genetic markers are used.

Hastalığı, Alzheimer hastalığından sonra en sık görülen nörodejeneratif hastalıktır.
Son yıllarda genetik formlarının ve patojenik mutasyonların tanımlanması ile
Parkinson Hastalığının etiyopatogenezine ilişkin bilgiler artmıştır. Parkinson
hastalığında tanımlanan ilk gen SNCA genidir ve ilk nokta mutasyonlar da bu gende
tanımlanmıştır. LRRK2 geni G2019S mutasyonu da Parkinson Hastalığında en sık
gözlenen nokta mutasyondur. Çalışmamıza sporadik Parkinson Hastalığı tanısı
almış 83 olgu dahil edilmiştir. Parkinson Hastalığından sorumlu olduğu bilinen
SNCA genindeki A30P ve LRRK2 genindeki G2019S mutasyonlarının Eskişehir
bölgesinde tanı alan olgulardaki sıklığının belirlenmesi, tanısal amaçlı
kullanılabilirliği ve ayrıca elde edilen veriler ile moleküler etiyolojisi belirlenmiş
olan olgulara hastalığı ve mutasyonu ile ilgili genetik danışmanın verilmesi
amaçlanmıştır. Sekans analizi ile A30P mutasyonunun bulunduğu SNCA geni
ekzon 2 ve G2019S mutasyonunun bulunduğu LRRK2 geni ekzon 41 genomik
bölgeleri incelenmiş ve hastalarda herhangi bir nokta mutasyon ya da polimorfizm
saptanmamıştır. Çalışmamızda Eskişehir bölgesinde sporadik Parkinson Hastalığı
tanısı alan olgularda A30P ve G2019S mutasyonlarının sık görülmediği sonucuna
ulaşılmıştır.
Parkinson’s disease is the second most common
neurodegenerative disease after Alzheimer disease. Resently understanding genetic
forms and pathogenic mutations has been providing growing knowledges about
etiopathogenesis of Parkison’s disease. SNCA was the first gene with mutations
causing Parkinson’s disease. LRRK’ gene G2019S mutation is the most common
mutation causing Parkinson’s disease. In our study we investigate 83 patients with
sporadic Parkison’s disease. Aims of our study are to identify the frequency of the
SNCA A30P and LRRK2 G2019S mutations in Parkinson’s disease patients from
the Eskisehir region of Turkey, diagnostic utility of these mutations and to confer
genetic counselling to the mutation carier patients. SNCA exon 2 and LRRK2 exon
41 was investigated with direct squencing method. Neither any point mutation or
polymorphism was detected in the SNCA exon 2 or LRRK2 exon 41 from the
patients. Our findings suggest that the SNCA A30P and LRRK2 G2019S mutations
are not frequent in Turkish patients with Parkinson’s disease.

Amaç: Bu çalısmada, Türk meme kanserli kadın hastalarda (n=98) ve kontrol grubunda
(n=56), FGFR2 genindeki rs2981582 ve TNRC9 genindeki rs3803662
polimorfizmlerini taramak ve bu polimorfizmlerin meme kanseri riski ile iliskisini
belirlemek amaçlanmıstır.
Metaryal ve Metot: Çalısmada, Bursa Ali Osman Sönmez Onkoloji hastanesinde tanı
konulan 98 meme kanseri hastasından ve ailesinde meme kanseri hikayesi olmayan 56
kontrol bireyinden alınan periferik kan örnekleri kullanıldı. FGFR2 genindeki
rs2981582 ve TNRC9 genindeki rs3803662 SNP’lerinin belirlenmesi için PZR-RFLP
yöntemi kullanıldı.
Bulgular: Yapılan analiz sonucunda, 98 hastanın FGFR2 genotip dağılımı CC
(wildtype), CT (heterozigot) ve TT (polimorfik) genotipleri için sırasıyla, %39.8
(n=39), %49 (n=48), %11.2 (n=11) olarak bulundu. TNRC9 genotip dağılımı ise CC
(wildtype), CT (heterozigot) ve TT (polimorfik) genotipleri için sırasıyla, %41.8
(n=41), %49 (n=48), %9.1 (n=9) olarak bulundu. Hastalar ve kontrol grubu arasında her
iki polimorfizm için genotipik farklılık gözlenmedi (p>0.05). Östrojen reseptör
durumuna göre hastalar karsılastırıldığında her iki polimorfizmde T allel sıklığı dağılımı
açısından ER pozitif hastalar ER negatif hastalara göre istatistiksel olarak anlamlı
düzeyde yüksek bulundu (P<0.05). Meme kanseri hastalarının evre ve grad özellikleri
arasında yapılan karsılastırmalarda da genotip dağılımları arasında anlamlı bir farklılık
bulunmadı.
Sonuç: Bu çalısma Türk meme kanseri hastalarında, FGFR2 ve TNRC9 gen
polimorfizmlerinin arastırıldığı ilk çalısmadır. Meme kanseri riski ile FGFR2 ve
TNRC9 polimorfizmleri arasındaki bağlantının daha büyük populasyonlarda ve daha
ayrıntılı çalısılması meme kanserine yatkınlığı olan bireylerin belirlenmesine önemli
katkılar sağlayacaktır.
Purpose: The aim of the study was to determine the rs2981582 polymorphism exists in
FGFR2 gene and rs3803662 polymorphism exists in TNRC9 gene for Turkish breast
cancer patients (n=98) and healthy controls (n=56) and to investigate the relation
between breast cancer risk.
Materials and Methods: In this study, the peripheral blood samples were used which
were obtained from both 98 breast cancer patients and healthy controls whom were
diagnosed in Bursa Ali Osman Sönmez Oncology Hospital. The PZR-RFLP method
was used to determine the rs2981582 polymorphism exists in FGFR2 gene and
rs3803662 polymorphism exists in TNRC9 gene.
Results: According to the analysis that have been made, the genotypic distribution for
FGFR2 of the 98 patients for CC (wildtype), CT (heterozygote) and TT (polymorphic)
genotypes were found to be respectively, % 39,8 (n=39),%49 (n=48) and %11,2 (n=11).
The genotypic distribution for TNRC9 of the 98 patients for CC (wildtype), CT
(heterozygote) and TT (polymorphic) genotypes were found to be respectively, % 41,8
(n=41), %49 (n=48) and %9,1 (n=9).No genotypical difference was observed for both
polymorphisms between patients and healthy controls (p>0,05). As the patients were
compared according to ER status, for both polymorphisms, statistically ER positive
patients were more significant than ER negative patients according to T allel frequency
(P<0.05). No signicant difference was observed between the genotypical distrubution
during the comparation of stage and grade characteristics of breast cancer patients.
Conclusion: This is the first study investigating the FGFR2 and TNRC9 gene

Mide kanseri, diyet ve Helicobacter pylori (Hp) gibi çevresel faktörlerden kaynaklanan genotipik değişimlerin progresif birikimi sonucu oluşmaktadır. Epigenetik mekanizmalardan biri olan promoter CpG adacık hipermetilasyonu, genin sessizleşmesine neden olmaktadır. Mide karsinomu aberan CpG hipermetilasyonun en sık gözlendiği kanser türlerinden biridir. Hp enfeksiyonu mide kanseri, kronik gastrit, tekrarlayan peptik ülser, atrofik gastrit ve intestinal metaplazi gelişiminde bir risk faktörüdür. Hp eradikasyonu sonucu bazı genlerde hipermetilasyon oranı azalmaktadır. Ancak bu oran bazı genlerde aynı kalmakta veya daha da artmaktadır. Bu çalışmada, 23 Hp(+) ve 14 Hp(-) olmak üzere toplam 37 gastrit hasta doku örneğinde ve 30 mide kanseri hasta doku örneğinde CYP1B1, SCGB3A1, MT1G, BCL2, P16, hMSH2 ve hMLH1 genlerinin promoter bölgelerinin metilasyon paternlerinin Metilasyon Sensitif -High Resolution Melting (MS-HRM) tekniği ile karşılaştırılması amaçlanmıştır. Ayrıca Hp(+) gastrit olgularının medikal eradikasyon tedavisinin genlerin metilasyon paternlerine etkisini de belirlemek amacıyla Hp(+) gastrit örneklerinin tedavi öncesi ve sonrası metilasyon paternleri ve oranları karşılaştırılmıştır. İncelenen mide kanseri hasta grubunda hMLH1, BCL2 ve CYP1B1 genlerine ilişkin metilasyon sıklıklarının gastrit hasta grubuna göre önemli düzeyde arttığı; MT1G, hMSH2 ve hMLH1 genlerinin metilasyon oranlarının Hp(+) gastrit olgularındaki medikal eradikasyon tedavisi sonrası azaldığı belirlenmiştir. Diğer taraftan kanser hasta grubunda hipermetile olan gen sayısının gastrit hasta grubuna göre önemli düzeyde arttığı ortaya konmuştur. Sonuç olarak MS-HRM tekniği metilasyon heterojenitesi gösteren tümör örneklerinde başarıyla uygulanabilecek bir yöntemdir. Daha büyük araştırma gruplarında klinikopatolojik verilerle birlikte değerlendirmelerin yapılmasının, etkin tedavi protokollerinin çok iyi gelişemediği bu kanser tipinin erken tanısı, prognoz değerlendirmesi için çok önemli olacağı sonucuna varılmıştır.
Gastric cancer is considered to be the result of a progressive accumulation of genotypic changes due to an adverse environment (i.e., diet and Helicobacter pylori infection). CpG island hypermethylation is closely associated with gene inactivation. Gastric carcinoma is one of the tumors with a high frequency of aberrant CpG island hypermethylation. Helicobacter pylori (Hp) infection is a risk factor for the development of gastric cancer, chronic gastritis, recurrent peptic ulcer disease, atrophic gastritis and intestinal metaplasia. By the medical eradication of Hp, the methylation rate of some genes might be reduced but the others remained hypermethylated or increased. This study was aimed to evaluate the promoter methylation status of the genes CYP1B1, SCGB3A1, MT1G, BCL2, P16, hMSH2 and hMLH1 in Hp (+) / (-) gastritis and gastric cancer tissues. Besides, the methylation profiles of these genes were also determined following the medical eradication treatment of Hp (+) in gastric cases. MS-HRM technique was performed in tissue samples of 37 acute gastritis and 30 gastric cancer. Statistical analyses were performed through SPSS 15 software. The frequencies of aberrant methylation profiles of hMLH1, BCL2 and CYP1B1 genes were significantly higher in gastric cancer group. Following the medical eradication treatment, the methylation levels of MT1G, hMSH2 and hMLH1 genes were decreased. When the number of methylated genes in gastritis and gastric cancer groups were compared, the number of methylated genes was significantly higher in gastric cancer group. The MS-HRM was determined as a powerful technique for the analysis of methylation profiles of genes in the heterogenous tumor tissues. Our findings show differential gene methylation in tumoral tissue, which allows us to conclude that hypermethylation is associated with gastric carcinogenesis. Further studies in larger groups with clinicopathological features of the cases should be performed to determine molecular biomarkers for early detection and prognostic evaluation of the cases with gastric cancer.

Gliomalar beyin dokusundan kaynaklanan tümörler arasında en sık gözlenen olup histopatolojik olarak çeşitli alt grupları ve malignite dereceleri vardır. Dünya Sağlık Örgütü Sınıflamasına göre Glioblastoma Multiforme (GBM) (evre IV), gliomalar arasında en malign olanı ve en sık görülenidir. Tümörü oluşturan genetik mekanizmalar henüz açıklığa kavuşturulamamıştır.Son yıllarda beyin tümörü dokusunun fenotipik özelliklerinin yanı sıra genetik özelliklerinin de önemli olacağı düşünülmekte ve bununla ilgili birçok moleküler sitogenetik (FISH) ve moleküler analizler (Array CGH, Southern blot vb. ) kullanılarak yeni araştırmalar yapılmaktadır.Bu çalışmada Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Nöroşirurji Anabilim Dalı'na başvuran ve Patoloji Anabilim Dalı tarafından histopatolojik olarak incelenerek ?Glioblastoma Multiforme? tanısı alan 40 olgu çalışılmıştır. Bu çalışmada alınan doku örneklerinde moleküler sitogenetik (FISH) analiz yöntemi kullanılarak, son yıllarda GBM hastalarında prognozun belirlenmesinde etkisi olduğu düşünülen ve üzerinde çalışılan genlerden MDM2 ve CDK4 genleri için amplifikasyon ve RB1 geni için delesyon paternlerinin saptanması amaçlanmıştır.Çalışmaya dahil edilen toplam 40 GBM'li olgunun 26'sı(%65) erkek, 14'ü(%35) kadın, olguların yaş ortalaması 55,45± 2,25, sağ kalım süresi ortalaması 11,17±1,07 olarak tespit edilmiştir.Bu çalışmada incelenen primer GBM'li olgular; %62,5'inde (25/40) MDM2 gen bölgesinde amplifikasyon, %60'ında (24/40) CDK4 gen bölgesinde amplifikasyon ve %47,5 (19/40) RB1 gen bölgesinde delesyon saptanmıştır.Türkiye'de Glioblastoma Multiforme'li olgularla yapılan bir çalışmaya rastlanılmamış olup CDK4 ve RB1 gen bölgeleri için Array CGH ve Southern blot yöntemleri kullanılmıştır. Bizim çalışmamız, GBM'li olgularda FISH yöntemi uygulanarak MDM2, CDK4 ve RB1 genlerinin incelendiği ilk çalışma olma özelliğini taşımaktadır.
Cause of brain tissue Glioblastoma Multiforme (GBM) the most observe tumour between all tumours and being histopatologic it has different under groups and malignite degree. Acording to World Health Organization Glioblastoma Multiforme is the most observe and the most being malign between GBM?s and not yet the genetic mechanizms which creat the tumour can be explain.İn the last years it is considered not only phenotypic characteristics of brain tumours tissue but olso genetic characteristics will be important in this subject, new researches are being done with using lots of molecular cytogenetic (FISH) and molecular analiysis. In our study we examined 40 patients that applied Neurosurgery Department in Eskisehir Osmangazi University Faculty of Medicine and histopathologically diagnosed as ?Glioblastoma Multiforme? by the Pathology department. In this study we aim to determine amplification of MDM2, CDK4 and RB1 gene deletion patterns which are supposed to be markers for the prognosis in patients with GBM in recent years via molecular cytogenetic methods (FISH).Total of 40 patients with GBM occured from 26 male (65%), 14 female (35%), the age average of the patients 55,45±2,25 and the survival period average is determined to be as 11,17±1,07.In our study the cases including primer GBM; in 62,5% (25/40) amplification in MDM2 gene region, in 60% (24/40) amplification in CDK4 gene region and 47,5% (19/40) deletion in RB1 gene region.There is a study in Turkey that done with Glioblastoma Multiforme patients by Arrey CGH and Southern bloting for detecting CDK4 and RB1 genes. Therefore, this present study can be seen as the first study which examines the MDM2, CDK4 and RB1 genes together with using FISH methods in the cases of GBM.

Giriş: Klonal myeloproliferatif bir hastalık olan kronik myeloid lösemi (KML), sitogenetik olarak 9. ve 22. kromozomlar arasındaki translokasyon ile karakterizedir. Philadelphia pozitif (Ph+) KML olgularının %10-15'inde yeniden düzenlenme sonucu oluşan der(9) kırık noktası olan ASS (9q34) gen bölgesinde büyük submikroskobik delesyonlar bildirilmiştir. Prognostik önemi tartışmalı olan ASS (9q34) delesyonları, klasik kemoterapi ve IFN-alfa tedavisi alan olgularda düşük oranda tedavi yanıtı ve kötü prognoz ile ilişkilendirilmiştir. Imatinib kullanımı sonrası literatürde yer alan az sayıdaki çalışmada olgular geç kronik fazda değerlendirilmiştir. Bu çalışmalarda ASS delesyonlarının prognostik önemine ait çelişkili sonuçlar elde edilmiş olmakla birlikte erken kronik faz KML olgularına ait yeterli veri mevcut değildir.Amaç: Çalışmamızda Ph+ KML olgularında ASS gen bölgesi delesyon oranının saptanması ve prognostik etkisinin değerlendirilmesi amaçlanmıştır.Gereç ve Yöntemler: Çalışmamıza 2001-2010 arasında KML tanısı almış ve Ph kromozomu tanımlanmış yaş dağılımı 22-66 arasında olan toplam 30 olgu dahil edildi. Olguların 23'ü yeni tanı aşamasında çalışmaya alınmış olup 22'sine imatinib, 1' ine hidroksiüre tedavisi verilmiştir. Kalan 7 olgu takip aşamasında çalışmaya alınmış 6'sına imatinib, 1'ine hidroksiüre tedavisi verilmiştir. ASS (9q34) delesyonu saptanması için LSI 9q34 probu kullanılarak FISH analizi yapılmıştır. Çalışmaya dahil edilen olgularda 3.ayda hematolojik yanıt, 12. ayda sitogenetik yanıt ve 18. ayda moleküler yanıt değerlendirilmiştir.Bulgular: Yapılan FISH analizleri sonucu 30 Ph+ KML olgusunun 11'inde (%36.6) ASS delesyonu saptanmıştır. ASS delesyonu saptanan 11 hastanın 10'unda (10/11) hematolojik yanıt saptanmış fakat bu 11 hastanın 6'sında (6/11) sitogenetik/moleküler sitogenetik yanıt saptanmamıştır. ASS delesyonu saptanmayan 19 (19/30) hastadan 12.ayını tamamlayan 16 hastanın 13'ünde sitogenetik yanıt, 11'inde de moleküler yanıt görülmüştür.Sonuç: Çalışmamız sonucunda ASS delesyonu bulunan olgularımızda sitogenetik yanıt oranımızın daha düşük olduğu izlenmiştir.
Background: Chronic myeloid leukemia (CML) is a clonal myeloproliferative disorder characterized by presence of Philadelphia chromosome (Ph), resulting translocation between chromosome 9 and 22. Large, submicroscopic deletions have been reported the breakpoint of ASS (9q34) gene region on rearranged der(9) in %10-15 of patient with (Ph+) CML. ASS (9q34) deletions have been associated with lower response rates and with adverse outcome in CML treated patients with conventional chemotherapy and interferon alfa (IFN-?). In studies following the usage of imatinib a few reported cases were evaluated at the chronic phase. In these studies, contradictory results were obtained about the importance of ASS (9q34) deletions yet not enough information were available regarding early chronic phase of CML cases.Aim: Detecting deletion of ASS (9q34) gene region rates and its prognostic effects.Material and methods: In our study, 30 patients with Ph+ CML were included between 2001-2010. Patient age is between 22-66. Twenty three of thirty patients are pretreatmant; 22 of 23 patients were treated with imatinib and 1 of 23 patients was treated with hydroxyurea. 7 of 30 CML patients were being followed-up, 6 of 7 patients were treated with imatinib and 1 of the cases treated with hydroxyurea. FISH analysis was performed for detection of ASS (9q34) deletions LSI 9q34 probe were used. Hematologic response at the end of 3rd month, cytogenetic response at the end of 12th month and molecular response at the end of 18 th month after initiation of treatment were evaluated in the cases.Findings: ASS (9q34) deletions were detected in 11 of 30 Ph+ CML patients in FISH analysis. Among cases who had ASS (9q34) deletions (n:11), 10 of them showed hematological response but 6 of them didn?t have cytogenetic/molecular response. 19 of 30 patients were found not having ASS (9q34) deletion. Among 16 patients who completed 12 months, 13 of them had cytogenetic response and 11 had molecular response.Results: In our study, lower cytogenetic response rates were detected in ASS (9q34) deleted cases.

Mesane kanseri, gelişmiş ülkelerde genitoüriner sistemde, prostat kanserinden sonra ikinci sıklıkla görülen kanserdir. Gelişmiş ülkelerde, mesane kanserlerinin %90'ından fazlasını ürotelyal orijinli DEK ( Değişici Epitel Karsinomu) oluşturur.Serin/treonin kinaz ailesinin üyesi olan Aurora kinazların hücre bölünmesi sırasında, iğ ipliklerinin oluşumu ve stabilitesi, sentrozom ayrılma ve olgunlaşması, kromozom kondensasyonu, ayrılması ve sitokinezde çeşitli rolleri vardır.Çalışmamızda AURKA ve AURKB genlerinin mesane kanserindeki değişimlerini incelemek, bu değişiklikleri tümör evre ve derecesiyle ilişkilendirmek ve mesane kanserlerinde progresyon için bir belirteç olup olmadıklarını değerlendirmek amaçlanmıştır.Çalışmamızda mesane kanseri tanısı almış 28 olgu ve 10 sağlıklı kontrol olgusundan alınan mesane yıkama sıvılarında AURKA ve AURKB genlerine ait değişimler ile CIN (kromozomal düzensizlik) durumu FISH yöntemi ile incelenmiştir.Olgularımızda, AURKA amplifikasyonlarının yüksek derece tümörler ve CIN ile birlikteliği anlamlı bulunmuştur. AURKA amplifikasyon miktarı patolojik evre ile korelasyon göstermiş fakat istatistiksel olarak anlamlı bulunmamıştır. Ancak çalışmamız sonucunda AURKA gen amplifikasyonlarının mesane kanseri progresyonunda önemli bir rol oynayabileceği ve bu amplifikasyonların mesane kanserli hastalarda prognozunu belirlemede bir marker olarak kullanılabileceği düşünülmektedir.AURKB genine ait incelemelerde, örneklerin %13'ünde delesyon gözlenmiş, tümör evre ve derecesiyle delesyonlar arasında istatistiksel bir anlam gözlenmemiştir. Ayrıca olgularımızın %20'sinde CEP 17 artışları tespit edilmiştir.Çalışmamız, Türk toplumunda Aurora kinazların mesane kanseri üzerinde etkisini ortaya koyan ilk çalışmadır. Çalışmamız ve bundan sonraki çalışmaların sonuçları, mesane kanseri ve Aurora Kinazlar arasındaki ilişkinin daha iyi anlaşılmasına, erken tanı ve tedavide yeni yaklaşımlar geliştirilebilmesine katkı sağlayacaktır.
Bladder cancer is the second most frequently diagnosed genitourinary tumor after prostate cancer. In developed countries, >90% of bladder cancers are (Transitional Cell Carcinoma) of urothelial origin.The Aurora A kinase belongs to serine/threonine group of kinases which are involved in centrosome separation and maturation, spindle assembly and stability, chromosome condensation, congression and segregation, and cytokinesis.The aim of our study was to investigate alterations of AURKA and AURKB genes; determine relationship between the alteration and tumor grade, stage and to assess their relevance as markers of progression in bladder cancers.In this study, FISH tests were used to examine AURKA, AURKB gene copy numbers and CIN (chromosomal instability) in bladder washing samples from 28 patients with bladder cancer and 10 healthy control subjects.As a result of our work, increased copy number of Aurora-A was linked to high grade tumor and CIN. Level of AURKA amplification was correlated with pathological stage but it was not statistically significant. Our study suggests that amplification of AURKA gene may play an important role in the progression of bladder cancer and that AURKA amplification for predicting the prognosis of bladder cancer in patients can be used as a marker.The results of AURKB gene analysis, deletion was observed in 13% of the samples for this gene. No statistically significant difference could be found between AURKB gene deletion and tumor grade, stage. Additionally, increased signals of CEP17 were found in 20% of cases.Our study is the first data demonstrating the effect on bladder cancer of Aurora Kinases in Turkish patients. The results of our study and many other studies contribute to the efforts to understand the association between bladder cancer and Aurora Kinases as well as to prospective diagnosis and treatment phases.

Meme kanseri, meme hücrelerinin anormal çoğalma gösterdikleri genetik bir hastalıktır. Çevreyle, yaşam biçimiyle ve kalıtımla ilişkilidir. Ülkemizde kadınlarda görülen kanserlerin %24,1'ini meme kanserlerinin oluşturduğu belirtilmektedir. DNA metilasyonu, kanserlerin gelişiminde rol oynayan epigenetik mekanizmalardan biridir. Özellikle promotor bölgedeki CpG adacıklarının aşırı metilasyonunun farklı kanser tiplerinde tümör baskılayıcı genlerin susturulmasında ana mekanizmalardan biri olduğu bilinmektedir. Bu çalışmada BRCA1 geni mutasyonu olmayan 30 meme kanseri olgusunun tümör örnekleri ile periferik kan örneklerinde BRCA1 gen metilasyon değişimlerinin incelenmesi, meme kanserinde kullanılan tümörün derecesi, östrojen ve progesteron reseptörleri gibi bazı biyolojik belirteçler ile karşılaştırılarak aralarındaki ilişkilerin ortaya konması hedeflenmiştir. İstanbul Üniversitesi İstanbul Tıp Fakültesi Genel Cerrahi Anabilim Dalı'nda meme kanseri tanısı almış 30 hastanın hem periferik kan DNA'ları ve hem de meme kanserli tümör DNA örnekleri ile aile öykülerinde meme kanseri olmayan 30 kontrol bireyin kan örneklerinde BRCA1 geni metilasyon profilleri ?Methylation Sensitive High Resolution Melting? (MS-HRM) yöntemi ile araştırılmıştır. İncelenen 30 kontrol bireyi örneklerinde aşırı BRCA1 metilasyonu gözlenmezken, 30 tümör örneğinde %60, periferik kan örneklerinde ise %16,7 oranlarında BRCA1 promotor aşırı metilasyonu saptanmıştır. Kan dokusunda metilasyon saptanan beş olgudan dördünün ileri evre meme kanseri olması dikkat çekiciydi. Araştırma grubunun periferik kan örneklerinde saptanan aşırı metilasyonun sirkülasyondaki serbest tümör hücrelerinden değil, somatik doku ile ilişkili olabileceği düşüncesindeyiz. Ayrıca olguların yaşları, östrojen ve progesteron reseptör durumları ve tümör dereceleri ile BRCA1 geni aşırı metilasyonu karşılaştırılmıştır. Kullandığımız MS-HRM tekniğinin metilasyon paternlerine ilişkin taramalarda yüksek doğruluk, zaman ve emek tasarrufu açısından uygulanabilir olduğu sonucuna varılmıştır.
Breast cancer is a type of cancer originating from abnormal proliferation of cells in breast tissue. It occurs because of an interaction between the environment, life style and defective genes. Worldwide, breast cancer comprises 10.4% of all cancers in women and the fifth most common cause of cancer-related death in women. In Turkey, breast cancer constitute of 24,1% of all cancers. DNA methylation is one of the epigenetic mechanisms that plays major role in cancer development and progression. The aberrant methylation of CpG islands of promoters of tumor suppressor genes has been shown to be involved in silencing of the genes in various cancer types.This study was aimed to compare methylation patterns of BRCA1 gene in breast tumor tissues and peripheral blood samples of 30 breast cancer cases and to determine its correlation with clinico-pathological characteristics of the cases. Here, we used the methlyation-sensitive high-resolution melting approach (MS-HRM) to examine the methylation status of the BRCA1, gene in tumor tissues and PBLs of the group of women diagnosed with breast cancer, and a control group with no signs of breast cancer. No aberrant methylation patterns was detected in control individuals whereas 60% and 16.7% of the tumor tissues and PBLs, respectively, had aberrant hypermethylation patterns in varios levels.It was so interesting that four of five cases in whom hypermethylation was detected in their PBLs had advanced stage (GIII) tumors in their histopathological diagnosis.Moreover, the BRCA1 aberrant methylation patterns of each case were compared with age, ER and PR status and tumor grades. This study was also confirmed that the Methlyation-Sensitive High-Resolution Melting approach (MS-HRM) is an usable, time and labour saving technique for methylation screenings.

Gelişmiş ülkelerde, mesane kanserlerinin %90'ından fazlasını ürotelyal orijinli DEK ( Değişici Epitel Karsinomu) oluşturur.Serin/treonin kinaz ailesinin üyesi olan Aurora kinazların hücre bölünmesi sırasında, iğ ipliklerinin oluşumu ve stabilitesi, sentrozom ayrılma ve olgunlaşması, kromozom kondensasyonu, ayrılması ve sitokinezde çeşitli rolleri vardır.Çalışmamızda AURKA ve AURKB genlerinin mesane kanserindeki değişimlerini incelemek, bu değişiklikleri tümör evre ve derecesiyle ilişkilendirmek ve mesane kanserlerinde progresyon için bir belirteç olup olmadıklarını değerlendirmek amaçlanmıştır.Çalışmamızda mesane kanseri tanısı almış 28 olgu ve 10 sağlıklı kontrol olgusundan alınan mesane yıkama sıvılarında AURKA ve AURKB genlerine ait değişimler ile CIN (kromozomal düzensizlik) durumu FISH yöntemi ile incelenmiştir.Olgularımızda, AURKA amplifikasyonlarının yüksek derece tümörler ve CIN ile birlikteliği anlamlı bulunmuştur. AURKA amplifikasyon miktarı patolojik evre ile korelasyon göstermiş fakat istatistiksel olarak anlamlı bulunmamıştır. Ancak çalışmamız sonucunda AURKA gen amplifikasyonlarının mesane kanseri progresyonunda önemli bir rol oynayabileceği ve bu amplifikasyonların mesane kanserli hastalarda prognozunu belirlemede bir marker olarak kullanılabileceği düşünülmektedir.AURKB genine ait incelemelerde, örneklerin %13'ünde delesyon gözlenmiş, tümör evre ve derecesiyle delesyonlar arasında istatistiksel bir anlam gözlenmemiştir. Ayrıca olgularımızın %20'sinde CEP 17 artışları tespit edilmiştir.Çalışmamız, Türk toplumunda Aurora kinazların mesane kanseri üzerinde etkisini ortaya koyan ilk çalışmadır. Çalışmamız ve bundan sonraki çalışmaların sonuçları, mesane kanseri ve Aurora Kinazlar arasındaki ilişkinin daha iyi anlaşılmasına, erken tanı ve tedavide yeni yaklaşımlar geliştirilebilmesine katkı sağlayacaktır.
Bladder cancer is the second most frequently diagnosed genitourinary tumor after prostate cancer. In developed countries, >90% of bladder cancers are (Transitional Cell Carcinoma) of urothelial origin.The Aurora A kinase belongs to serine/threonine group of kinases which are involved in centrosome separation and maturation, spindle assembly and stability, chromosome condensation, congression and segregation, and cytokinesis.The aim of our study was to investigate alterations of AURKA and AURKB genes; determine relationship between the alteration and tumor grade, stage and to assess their relevance as markers of progression in bladder cancers.In this study, FISH tests were used to examine AURKA, AURKB gene copy numbers and CIN (chromosomal instability) in bladder washing samples from 28 patients with bladder cancer and 10 healthy control subjects.As a result of our work, increased copy number of Aurora-A was linked to high grade tumor and CIN. Level of AURKA amplification was correlated with pathological stage but it was not statistically significant. Our study suggests that amplification of AURKA gene may play an important role in the progression of bladder cancer and that AURKA amplification for predicting the prognosis of bladder cancer in patients can be used as a marker.The results of AURKB gene analysis, deletion was observed in 13% of the samples for this gene. No statistically significant difference could be found between AURKB gene deletion and tumor grade, stage. Additionally, increased signals of CEP17 were found in 20% of cases.Our study is the first data demonstrating the effect on bladder cancer of Aurora Kinases in Turkish patients. The results of our study and many other studies contribute to the efforts to understand the association between bladder cancer and Aurora Kinases as well as to prospective diagnosis and treatment phases.

Meme kanseri, meme hücrelerinin anormal çoğalma gösterdikleri genetik bir hastalıktır. Çevreyle, yaşam biçimiyle ve kalıtımla ilişkilidir. Ülkemizde kadınlarda görülen kanserlerin %24,1'ini meme kanserlerinin oluşturduğu belirtilmektedir. DNA metilasyonu, kanserlerin gelişiminde rol oynayan epigenetik mekanizmalardan biridir. Özellikle promotor bölgedeki CpG adacıklarının aşırı metilasyonunun farklı kanser tiplerinde tümör baskılayıcı genlerin susturulmasında ana mekanizmalardan biri olduğu bilinmektedir. Bu çalışmada BRCA1 geni mutasyonu olmayan 30 meme kanseri olgusunun tümör örnekleri ile periferik kan örneklerinde BRCA1 gen metilasyon değişimlerinin incelenmesi, meme kanserinde kullanılan tümörün derecesi, östrojen ve progesteron reseptörleri gibi bazı biyolojik belirteçler ile karşılaştırılarak aralarındaki ilişkilerin ortaya konması hedeflenmiştir. İstanbul Üniversitesi İstanbul Tıp Fakültesi Genel Cerrahi Anabilim Dalı'nda meme kanseri tanısı almış 30 hastanın hem periferik kan DNA'ları ve hem de meme kanserli tümör DNA örnekleri ile aile öykülerinde meme kanseri olmayan 30 kontrol bireyin kan örneklerinde BRCA1 geni metilasyon profilleri ?Methylation Sensitive High Resolution Melting? (MS-HRM) yöntemi ile araştırılmıştır. İncelenen 30 kontrol bireyi örneklerinde aşırı BRCA1 metilasyonu gözlenmezken, 30 tümör örneğinde %60, periferik kan örneklerinde ise %16,7 oranlarında BRCA1 promotor aşırı metilasyonu saptanmıştır. Kan dokusunda metilasyon saptanan beş olgudan dördünün ileri evre meme kanseri olması dikkat çekiciydi. Araştırma grubunun periferik kan örneklerinde saptanan aşırı metilasyonun sirkülasyondaki serbest tümör hücrelerinden değil, somatik doku ile ilişkili olabileceği düşüncesindeyiz. Ayrıca olguların yaşları, östrojen ve progesteron reseptör durumları ve tümör dereceleri ile BRCA1 geni aşırı metilasyonu karşılaştırılmıştır. Kullandığımız MS-HRM tekniğinin metilasyon paternlerine ilişkin taramalarda yüksek doğruluk, zaman ve emek tasarrufu açısından uygulanabilir olduğu sonucuna varılmıştır.
Breast cancer is a type of cancer originating from abnormal proliferation of cells in breast tissue. It occurs because of an interaction between the environment, life style and defective genes. Worldwide, breast cancer comprises 10.4% of all cancers in women and the fifth most common cause of cancer-related death in women. In Turkey, breast cancer constitute of 24,1% of all cancers. DNA methylation is one of the epigenetic mechanisms that plays major role in cancer development and progression. The aberrant methylation of CpG islands of promoters of tumor suppressor genes has been shown to be involved in silencing of the genes in various cancer types.This study was aimed to compare methylation patterns of BRCA1 gene in breast tumor tissues and peripheral blood samples of 30 breast cancer cases and to determine its correlation with clinico-pathological characteristics of the cases. Here, we used the methlyation-sensitive high-resolution melting approach (MS-HRM) to examine the methylation status of the BRCA1, gene in tumor tissues and PBLs of the group of women diagnosed with breast cancer, and a control group with no signs of breast cancer. No aberrant methylation patterns was detected in control individuals whereas 60% and 16.7% of the tumor tissues and PBLs, respectively, had aberrant hypermethylation patterns in varios levels.It was so interesting that four of five cases in whom hypermethylation was detected in their PBLs had advanced stage (GIII) tumors in their histopathological diagnosis.Moreover, the BRCA1 aberrant methylation patterns of each case were compared with age, ER and PR status and tumor grades. This study was also confirmed that the Methlyation-Sensitive High-Resolution Melting approach (MS-HRM) is an usable, time and labour saving technique for methylation screenings.

Genomun bütünlüğü DNA tamir mekanizması ile korunur. DNA tamir genlerinde oluşan polimorfizmler DNA tamir mekanizmasını bozarak hematolojik malignitelerde risk faktörü oluşturmaktadır. Xeroderma pigmentosum Complementetion group D (XPD) geni nükleotid kesip-çıkarma tamir (NER) mekanizmasında görev alır. XPD geni kodon 312 polimorfik bölgesindeki polimorfizm DNA tamir kapasitesini etkiler.Amaç: Bu çalışmada, Türk populasyonunda XPD kodon 312 geni polimorfizmi ile prostat kanseri arasındaki ilişkiyi araştırmak amaçlanmıştır.Gereç-Yöntem: Prostat kanserli 100 hasta ve 150 kontrol DNA'sının XPD kodon 312 lokusu PCR ile çoğaltılmıştır. Elde edilen PCR ürünleri ARMS yöntemi ile analiz edilmiştir.Bulgular: Yapılan çalışma sonucunda, XPD kodon 312 genotip frekansları hastalarda %52 GG, %32 GA, %16 AA; kontrollerde %64 GG, %24 GA, %12 AA olarak saptanmıştır. G allel frekansı hastalarda %70, kontrollerde ise %76 olarak bulunmuştur. A allel frekansı hastalarda %30, kontrollerde %24 olarak saptanmıştır.Sonuç ve Tartışma: Türk populasyonunda XPD kodon 312 genotip dağılımlarının prostat kanserli hastalar ve kontrollerde istatiksel olarak önemli bir farklılık göstermediği (P>0.05) saptanmıştır. Elde edilen veriler XPD kodon 312 polimorfizminin prostat kanseri tanısında genetik markır olarak kullanılmaya uygun olmadığı görülmüştür.
Completesness of genom is protected by repair mechanism of DNA. The polymorphism which comprise of DNA repair genes constitute of risk factor in hematologic malignity while ruining the DNA repair mechanism. Xeroderma pigmentosum Complementetion group D (XPD) gene take charge in nucleotide excision repair mechanism(NER). XPD gene effects polymorphism DNA repair capacity that takes part in codon 312 polymorphic area.Aim: In this study, the relationship between XPD codon 312 gene polymorphism in Turkish population and prostate cancer is purposed.Methods:100 patient with prostate cancer and 150 control DNA's XPD codon 312 locus wre augmented with PCR. Acquired PCR products wre analysed with ARMS method.Findings: As a result of the study XPD codon 312 genotype frequencies were detected as %52 GG, %32 GA, %16 AA on patient; and %64 GG, %24 GA, %12 AA on controls. G allel frequency was found %70 on patient and %76 on controls. A allel frequency was detected %30 on patient and %24 on controls.Result and Conlusion: It is detected that the dispersion of XPD codon 312 genotype on prostate cancer patient and controls do not reflect a significant difference (P>0.05). Accoring to acquired data it is seen that using XPD codon 312 polymorphism as a genetic marker on diagnostic prostate cancer is not suitable.

Gebeliğin 20. haftasından önce embriyonun komplet ya da inkomplet olarak, uterustan spontan atılması yoluyla gebeliğin sonlanmasına abortus adı verilmektedir. Habituel abortus ya da tekrarlayan gebelik kaybı (TGK) art arda iki ya da daha fazla gebeliğin abortus ile sonuçlanmasıdır. Genel popülasyonda ilk trimester gebeliklerin yaklaşık %10-%15’i spontan abortus ile sonlanırken %1-2’si TGK olarak gözlenmektedir. Genel olarak TGK’nın etiyolojisinde %2-5 arasında genetik nedenler sorumludur. İlk trimester TGK’nın %60’ı, 2. Trimester TGK’nın %10-15’i, 3. trimester ölü doğumların %5’inden kromozom anomalileri sorumlu tutulmaktadır.
Retrospektif olarak planladığımız bu çalışmamızda; Ocak 2008 ve Aralık 2016 tarihleri arasında, tekrarlayan gebelik kayıpları nedeniyle Eskişehir Osmangazi Üniversitesi Tıp Fakültesi Tıbbi Genetik Anabilim Dalına başvuran veya yönlendirilen çiftlerde gerçekleştirilen konvansiyonel sitogenetik ve moleküler sitogenetik analiz verilerinin aile ve obstetrik öyküleri ile birikte değerlendirilmesi amaçlanmıştır. İki ya da daha fazla tekrarlayan düşük hikayesi olan 969 çift, toplam 1938 olgu klinik, sitogenetik ve moleküler sitogenetik verileriyle birlikte değerlendirilmiştir.
Sitogenetik analiz sonucunda örneklerin 1779‘unda (%91,7) normal kromozom kuruluşu, 46’sında (%2,3) anormal kromozom kuruluşu ve 113’ünde (%5,8) kromozom polimorfizmi saptanmıştır.
Anormal kromozom kuruluşu saptanan olguların 44’ünde yapısal anomali, 2 olguda sayısal anomali saptanmıştır. Yapısal anomaliler içinde 34 olguda resiprokal translokasyon, 6 olguda robertsonian translokasyon, 2 olguda marker kromozom ve birer olguda kromozom 12’nin inversiyonu ile kromozom 15’in uzun kolunda yeniden düzenlenme saptanmıştır.
Çalışmamız bölgemizdeki tekrarlayan gebelik kayıplarının insidansının saptanması açısından önemli olup, ebeveynlere yapılan analizler sonucu genetik etiyolojinin belirlenmesinin çiftlerin sonraki gebeliklerinin prognozu açısından yol gösterici olacağı düşünülmektedir.
Before the 20th week, spontaneous termination of pregnancy, either complete or incomplete evacuation of embrio from the uterus, is named as abortion. Habituel abortion or recurrent pregnancy loss is termination of, at least two or more consecutive pregnancies with abortion. In general population, 10-15% of pregnancies in first trimester results with abortion and 1-2% of those are habituel abortion. Genetic factors are responsible for 2-5% of the aetiology in habituel abortions. Chromosomal abnormalities are responsible for the 60% of the habituel abortions in the first trimester, 10-15% of the habituel abortions in second trimester and 5% of the prenatal deaths in third trimester.
This retrospective study is performed between January 2008 and December 2016 in Eskisehir Osmangazi University Faculty of Medicine, Department of Medical Genetics to evaluate cytogenetic and molecular cytogenetic analyzes in couples with repetitive pregnancy loss. A total of 1938 cases, 969 couples with two or more recurrent pregnancy losses that applied to the Eskişehir Osmangazi University Health Practice and Research Hospital, Clinic of Medical Genetics, were evaluated clinically and cytogenetic and molecular cytogenetic methods were performed.
Cytogenetics analyses revealed that 1777 cases have normal chromosome constitutions whereas 46 cases have abnormal karyotypes and 113 cases have chromosomal polymorphisms.
Among the patients with abnormal chromosomal organisations; 44 cases have structural abnormalities and 2 cases had numerical abnormalities. In the structural abnormalites, there were 34 patients with reciprocal translocations, 6 with robertsonian translocations, 2 patients with marker chromosomes, 1 patient with inversion of the chromosome 12 and 1 patient with rearrengement of the long arm of the chromosome 15.
Our study has an important role in detecting the incidence of the recurrent pregnancy losses in our area and the significance of cytogenetic analysis in the explanation of recurrent fetal losses is revealed once again.

Primer kardiyomiyopatiler, kalp kasında meydana gelen
bozukluklardan ortaya çıkan herhangi bir disfonksiyona sekonder gelişmeyen
ve büyük ölçüde genetik altyapısı bulunduğu düşünülen bir hastalık grubudur.
Kalp yetmezliğinin ve ani ölümün en önemli nedenlerinden birisi olmasına
rağmen, kalp transplantasyonu haricinde mevcut tedaviler,
kardiyomiyopatilerde semptomların hafifletilmesi üzerine kuruludur. Son
yıllarda genetik varyasyona yönelik tanı ve tedaviler göz önünde
bulundurulduğunda ise, mitokondriyal DNA (mtDNA) birçok araştırmanın
önemli bir konusu olmuştur. Mitokondriyal DNA üzerinde saptanan
varyantların bir kısmı enerji ihtiyacı yüksek kalp, beyin gibi organlardaki hem
yapısal hem de fonksiyonel sorunlarla ilişkilendirilmiştir. Dolayısıyla
kardiyomiyopatiye neden olduğu bilinen miyokard bazında meydana gelen
bozuklukların, çoğunlukla biyoenerjetik yolaklardaki mitokondriyal
etkileşimlerden kaynaklı olabileceği düşünülmektedir. Ancak bu yaklaşımla
yapılan sınırlı sayıda çalışma bulunmaktadır. Ayrıca çekirdek DNA’sından
(nDNA) eksprese olan mitokondriyal proteinlerin, mtDNA’dan direkt eksprese
olanlarla olan ilişkisi de son zamanlarda oldukça fazla odak haline
getirilmiştir.
Çalışmamız iki aşamadan oluşmaktadır. İlk aşamada 27 pediatrik
kardiyomiyopati hastasında, kardiyomiyopati üzerinde direkt etkisi olduğunu
veya hastalığın progresyonuna katkısı olduğunu düşündüğümüz
kardiyomiyopati ile ilişkilendirilmiş 19 gen (ACTA2, ACTC1, ACTN2, CALR3,
CAV1, CAV3, PRKAG2, SLC12A3, SLC19A2, SLC25A4, SLC22A5, SLC25A20,
SLC2A10, SLC2A11, SLC52A2, SLC6A2, TNNC1, TNNI3, TNNT2) yeni-nesil
sekanslama (NGS) yöntemi kullanılarak incelenmiştir. İkinci aşamada ise 27
pediatrik kardiyomiyopati ve 31 sağlıklı pediatrik bireyden oluşan kontrol
grubunun tüm mtDNA sekanslanması yine NGS yöntemiyle
gerçekleştirilmiştir. Totalde 58 bireyin mtDNA sekanslamasının yapıldığı

çalışmamız, aynı zamanda etnik/bölgesel haplogrup varyasyonlarının
yorumlanması ile tüm mtDNA’nın sekanslanması ile birlikte Türkiye mtDNA
haplogrup veri haritasına bu denli katkıda bulunmayı amaçlayan ilk çalışma
olmuştur.
Çalışmamızın ilk kısmında, 5 hastada CAV3, TNNC1, TNNT2 ve
SLC22A5 genlerinde önemli veya önemli olduğu düşünülen varyantlar tespit
edilmiştir. Bunu takiben mtDNA analizleri kontrol grubuyla karşılaştırılarak
yapıldıktan sonra; saptanan mtDNA varyantlarının analizi neticesinde
toplamda birbirinden farklı 264 varyant arasından anlamlı olduğu düşünülen
41 adet; ikinci bir filtreleme sonrasında ise 20 varyant (%7,58) saptanmış ve
önemleri tartışılmıştır. Ayrıca sol ventriküler noncompaction
kardiyomiyopatisi olan bir olguda saptanan MT-RNR1 genindeki tek nükleotid
varyasyonu (SNV) olan m.684T>C ise bilgimiz dahilinde novel bir varyant
olarak tespit edilmiştir.
Karmaşık etyopatogenezi açısından kardiyomiyopati ve karmaşık
analizi olan mtDNA varyantları bu çalışmada birlikte değerlendirilmiştir. Bu
çalışma sonucunda kardiyomiyopati gibi mitokondriyal defektlere bağlı
geliştiği düşünülen hastalıkların nDNA’daki hedef genlerinde incelemesi
gerçekleştirildikten sonra, hastalıkların ana nedeni olmasa bile
ekspresyonunu ve penetransını değiştirebileceği yönündeki bilgilerden yola
çıkarak bu hastalarda mtDNA dizilemesini, etiyolojiyi açıklayabileceğini
düşündüğümüz nadir varyantlar saptadığımız için önermekteyiz. Ayrıca
istatistiksel anlamlılığı artırmak için daha büyük popülasyonlarda yapılan
çalışmalara ihtiyaç duyulduğu sonucuna varılmıştır. Böylece bölgemizdeki
mitokondriyal çeşitliliği daha iyi tanımlayarak ve biyoinformatik çalışmalarla
uygun veri tabanları hazırlayarak hastalıkların tanısı, tedavisi veya
önleyiciliği açısından yönlendirilmesine katkı sağlayabiliriz
Primary cardiomyopathies are a group of disease caused by defects in
myocardium and do not develop as secondary to another disfunction occur in
myocardium, which are thought hugely related to genetic fundamentals. Even
though cardiomyopathy is one of the leading causes of heart failure and
sudden cardiac death among the young all over the world, present therapeutic
approaches are used mostly to relieve the symptoms of the disease except
heart transplantation. Indeed, considering treatments regarding genetic
variations to offer a permanent solution, mitochondrial DNA (mtDNA) has
been one of the important subjects for recent researches. The variants detected
in mtDNA have been associated with both structural and functional
complications at organs with high energy demand such as heart and brain.
Defects in myocardium causing cardiomyopathy are thought to be derived from
mostly mitochondrial interactions with respect to bioenergetic pathways.
However, there is a limited number of researches on this subject. In addition,
researches recently have been focusing on the interaction of the mitochondrial
proteins expressed by both nuclear (nDNA) and mtDNA genes.
This study consists of two apporaches. In first part, our research was
started with the examination of 19 genes (ACTA2, ACTC1, ACTN2, CALR3,
CAV1, CAV3, PRKAG2, SLC12A3, SLC19A2, SLC25A4, SLC22A5, SLC25A20,
SLC2A10, SLC2A11, SLC52A2, SLC6A2, TNNC1, TNNI3, TNNT2) that are
thought to directly affect or contribute to the progression of cardiomyopathies
on 27 pediatric cases by next-generation sequencing (NGS). Next, whole
mtDNA of 27 patients was sequenced by NGS again to make a comparison
with a control group which is comprised of 31 individuals. In addition, this
study has been the first large scale attempt to contribute that much to
Turkey’s mtDNA ethnic/geographic-associated haplogroup variation thanks to
sequencing of fifty-eight person’s whole mtDNA.

In the first part of the study, 4 single nucleotide-variations (SNVs) that
are thought to be important were detected in 5 patients. Subsequently, after
first mtDNA variant analysis in comparison with control group was performed,
41 different SNVs were detected out of 264, of which 20 (20/264; %7,58) are
thought to be meaningful after second filtering variants by in silico
evaluations. Moreover, we have identified a homoplasmic m.684T>C in MT RNR1 gene in one patient with left-ventricular noncompaction
cardiomyopathy.
Two issues, which are cardiomyopaties with its complex
etiopathogenesis and mtDNA variants with its difficult interpretation, were
evaluated together within this study. We, therefore, suggest that mtDNA
variants should be examined along with other target gene panels since
mitochondria could alter the expressivity and the penetrance of the nDNA
variations and accordingly as we detected such rare variants that could
enlighten the etiology even if its effects are not the first cause of the
mitochondrial suspected diseases. On the other hand, more studies conducted
on large sample sizes should be carried out to improve the significance of the
statistical tests. We, hereby, could contribute much more to decision making

Kronik lenfositik lösemi (KLL) lenfoid seriden köken alan ve
karakteristik genetik değişikliklerin bulunduğu bir lösemi tipi olarak
tanımlanmaktadır. Kronik lenfositik lösemide genetik değişiklikler,
kromozomal aberasyonlar ve gen mutasyonlarıdır. Literatürlerde en
sık gözlenen kromozomal aberasyonlar ise 13. ve 11. kromozumun
uzun kol delesyonu (del13q, del11q), 17. kromozomun kısa kol
delesyonu (del17p) ve 12. kromozomun trizomisi olarak bildirilmiştir.
Belirtilen anomalilerden izole del13q iyi prognostik olarak kabul
edilmektedir. Fakat bazı literatürlerde 13. kromozomun uzun
kolundaki delesyonun büyüklüğü ile delesyona ek anomalilerin
varlığının prognozu etkilediği bildirilmiştir. Kronik lenfositik lösemi
belirtilen kromozomal aberasyonların yanında sıklıkla NOTCH1,
SF3B1, BIRC3, TP53 ve XPO1 genlerinde mutasyonlar saptanmıştır.
Belirtilen genler içinde NOTCH1 ve SF3B1 genlerinin hücre
siklüsünde, apoptoz ve RNA splicing gibi hücre işlevinde önemli
mekanizmalarda görevli olduğu bilinmektedir. Bu nedenle NOTCH1
ve SF3B1 genlerindeki mutasyonların hastalığın patogenezinde
önemli etkilerinin olması beklenilmektedir. Ek olarak literatürlerden
elde edinilen bilgiler doğrultusunda belirlediğimiz genlerin
hastaların tedaviye başlama süreleri ve sağkalım süresi üzerine
etkileri bulunduğu da bildirilmektedir. Biz de çalışmamızda FISH
yöntemiyle belirlediğimiz izole del13q saptanan kırk üç hastada kötü
prognostik etkisi olan NOTCH1 ve SF3B1 genlerinde mutasyon
profilini Sanger Sekanslama yöntemiyle inceledik. Belirtilen hasta
grubunda NOTCH1 genininde en sık görülen 7541_7542delCT
mutasyonunu bir hastada (%2.4) saptarken, SF3B1 geninde mutasyon
saptayamadık.

Çalışmamızda SF3B1 ve NOTCH1 genlerinde hasta sayısının ve
mutasyon oranlarının az olması nedeni ile del13q oranları, RB1 gen
delesyon varlığı, hastalık evreleri, tedaviye başlama süresi ve OS
parametreler ile ilişkisi istastiksel olarak değerlendirilememiştir.
Ancak olgu serimizde sadece 1 hastada NOTCH1 geninde mutasyon
bulunması; ilgili mutasyon ile del13q hasta grubu arasında bir
ilişkinin olmadığını düşündürmektedir. Yapılan literatür
değerlendirmesinde çalışmamız Türk populasyonunda FISH yöntemi
ile izole del13q saptanan KLL hastalarında ilgili genlerin mutasyonel
durumunu değerlendiren ilk çalışma olarak görülmekte ve bu yönü
ile literatüre katkı sağlamaktayız
Chronic lymphocytic leukemia (CLL) is defined as a type of
leukemia originating from the lymphoid series with characteristic
genetic changes. Genetic changes, chromosomal aberrations and gene
mutations in chronic lymphocytic leukemia. The most common
chromosomal aberrations in the literature have been reported as
deletion of the long arm of the 13th and 11th chromosomes (del13q,
del11q), deletion of the short arm of the 17th chromosome (del17p)
and trisomy of the 12th chromosome. Del13q isolated from the
mentioned anomalies is considered to be good prognostic. However, in
some literature, it has been reported that the size of the long arm of
chromosome 13 and the presence of additional anomalies affect the
prognosis. In addition to chromosomal aberrations with chronic
lymphocytic leukemia, mutations in the genes of NOTCH1, SF3B1,
BIRC3, TP53 and XPO1 are frequently detected. Among the mentioned
genes, NOTCH1 and SF3B1 genes are known to be involved in cell
cycle, apoptosis and important mechanisms in cell process such as
RNA splicing. Therefore, mutations in NOTCH1 and SF3B1 genes are
expected to have important effects on the pathogenesis of the disease.
In addition, it has been reported that the genes we identified in
accordance with the information obtained from the literature have
effects on the duration of treatment and survival time of the patients.
In our study, we investigated the mutation profile of NOTCH1 and
SF3B1 genes with poor prognostic effect in forty-three patients with
isolated del13q detected by FISH method by Sanger Sequencing
method. We detected 7541_7542delCT mutation in NOTCH1 gene in
one patient (2.4%), but we could not detect mutations in SF3B1 gene.

In our study, due to the low number of patients and mutation
rates in SF3B1 and NOTCH1 genes, del13q rates, presence of RB1
deletion, disease stages, duration of treatment and OS parameters
could not be evaluated statistically. However, in our case series, only
1 patient had mutations in NOTCH1 gene; suggest that there is no
relationship between the relevant mutation and the del13q patient
group. In the literature review, our study is seen as the first study
evaluating the mutational status of related genes in CLL patients with
isolated del13q detected by FISH method in Turkish population and
we contribute to the literature with this aspect

Meme kanseri, kadınlarda en sık gözlenen kanser tipidir ve kanser tanısı
almış her 4 kadından 1'inin tanısı meme kanseridir. Meme kanseri gelişimi için
yaş, sigara / alkol / radyasyon maruziyeti, obezite, üreme ve adet öyküsü ile
ilişkili risk faktörleri bulunmasının yanı sıra meme kanserine yatkınlık
oluşturan genetik risk faktörleri de söz konusudur. Meme kanseri vakalarının
yaklaşık %10-30'u kalıtsal faktörlere atfedilse de, vakalarının sadece %5-10'unda
güçlü kalıtsal bir bileşen tespit edilmiştir. Patojenik değişimleri ailevi meme
kanseri ile en sık ilişkilendirilmiş genler BRCA1 ve BRCA2 iken güncel bilimsel
bilgiler diğer bir grup genin patojenik değişimlerinin de meme kanserine
yatkınlıktan sorumlu olduğunu ortaya koymaktadır.
Çalışmamızda meme kanseri tanısı almış ve risk faktörleri
değerlendirildiğinde ailevi meme kanseri yatkınlığından şüphelenilmiş, fakat
BRCA1 ve BRCA2 genlerinde yeni nesil dizileme (YND) yöntemi ile herhangi bir
patojenik değişim saptanmayan otuz sekiz olguda meme kanserine yatkınlıktan
sorumlu diğer genlerde genetik incelemeler yapılması amaçlanmıştır. Bu amaçla
YND ile rutinde çalışılan ailesel kanser paneli içerisinde bulunan ATM, BARD1,
BRIP1, CDH1, CHEK2, MRE11, MSH6, NBN, PALB2, PTEN , RAD51, RAD51C,
STK11, TP53 genleri dizilenmiş ve tespit edilen varyantlar kılavuzlara uygun
olarak sınıflandırılarak patojenik (P), olası patojenik (OP) ve klinik önemi
belirsiz (KÖB) olarak sınıflandırılmış varyantlar değerlendirmeye alınmıştır.
Çalışma sonucunda otuz sekiz olgunun 8 (%21)'inde 9 adet P / OP veya
KÖB olarak sınıflandırılmış varyant tespit edilmiştir. Bu değişimlerin 4'ü ATM
geninde, 2'si BRIP1 geninde, 2'si CHEK2 geninde, 1'i MRE11 geninde, 1'i MSH6
geninde bulunmıştır. Çalışmamız sayesinde patojenik değişim tespit edilen
olgular ile aileleri koruyucu tedavi olanaklarına yönlendirilmiş, hangi genlerde
daha sık olarak patojenik değişimler bulunduğu tespit edilmiş ve literatürde

daha önce bildirilmemiş yeni varyantlar meme kanseri yatkınlığı ile
ilişkilendirilmiştir
Breast cancer is the most common cancer type in women and every 1 in 4
women diagnosed with cancer has breast cancer. There are several risk factors
for breast cancer development including age, cigarette / alcohol / radiation
exposure, obesity, conception and menstruation related risks, but in addition to
that, genetic factors are responsible for susceptibility to breast cancer. It is
estimated that 10-30% of breast cancer patients have hereditary risk factors but
high-risk genetic component is identified in only 5-10% of the patients.
Pathogenic variants of BRCA1 and BRCA2 genes are the most frequently
identified genetic risk factors in breast cancer, but recent scientific discoveries
demonstrates that pathogenic variants of other group of genes are causing
predisposition to breast cancer.
In this study, we planned to perform genetic analyses for those other group
of genes in thirty eight breast cancer patients with the suspicion of hereditary
breast cancer syndroms who were found to carry no mutations in BRCA1 and
BRCA2 genes by next generation sequencing (NGS) method. With this aim, ATM,
BARD1, BRIP1, CDH1, CHEK2, MRE11, MSH6, NBN, PALB2, PTEN , RAD51,
RAD51C, STK11, TP53 genes are sequenced by NGS and identified variants are
classified according to current guidelines. Variants classified as pathogenic (P),
likely pathogenic (LP) and uncertain significance (VUS) are included to the
results.
As a result 8 (21%) of the thirty eight patients were found to carry 9
variants classified as either P, LP or VUS. Among these variants, 4 were in ATM
gene, 2 were in BRIP1 gene, 2 were in CHEK2 gene,1 was in MRE11 gene, 1 was
in MSH6 gene. By this study, patients carrying pathogenic variants and their
family members are referred to preventive medicine opportunities, pathogenic
variant frequencies in related genes are identified and novel variants associated
with breast cancer are discovered

Kronik lenfositik lösemi yetişkinlerde en sık gözlenen lösemi
türü olup, sıklıkla tekrar eden bazı kromozomal anomalilerin
prognostik önemleri tanımlanmıştır. En sık gözlenen anomali
delesyon 13 (del(13q)) olup, izole göründüğünde iyi prognostik
biyobelirteç olarak kabul edilmektedir. Ancak izole del(13q) saptanan
KLL olgularının klinik bulgularının oldukça heterojen olduğu
gözlenmektedir. Bizim çalışmamızda izole del(13q) saptanan
olgularda del(13q) büyüklüğünün ve genomdaki diğer kopya sayısı
değişiklikleri (KSD) ve heterozigosite kayıplarının (LOH) klinik
heterojenite üzerindeki etkisinin araştırılması amaçlanmıştır.
Çalışmamıza FISH yöntemi ile izole del(13q) saptanan otuz iki
KLL olgusu dahil edilmiş olup, otuz olgunun periferik kan
örneklerinden elde edilen DNA örneklerinde aCGH+SNP yöntemi ile
KSD ve LOH’lar analiz edilmiştir.
Çalışmamız sonucunda 28/30 olguda del(13q) saptanmıştır.
Delesyon büyüklüğü 0,34 - 28,81 Mb arasında değişkenlik göstermiştir
ve delesyon tipi (tip ve tip 2 ) ile klinik bulgular arasında bir ilişki
saptanmamıştır. Olgularımızdan 1’i izole del(13q) iken kalan olguların
birinde del(13q) dışında 1 kromozom bölgesinde, 4 olguda 2 kromozom
bölgesinde KSD saptanmış olup, diğer olgularda en az 3 KSD tespit
edilmiştir. Bir olguda saptanan en fazla KSD altmış bir adettir.
Yaptığımız çalışma sonucunda del(13q) büyüklüğünün oldukça
heterojen olduğu ve içerdiği genler açısından da heterojenite
gösterdiği saptanmıştır. Genomda diğer KSD’ler arasında nadir
KSD’ler saptanmış olup, ayrıca 1 olguda ATM gen bölgesini içeren
LOH tespit edilmiştir. Saptanan KSD’ler arasında 16p13.3, NOTCH,
COL1A1 ve 2p artışlarının ve 18p11.32 kayıplarının prognostik
önemlerinin olabileceği sonucuna varılmıştır. Sonuç olarak, kopya
sayısı değişikliklerinin, KSD büyüklüklerinin ve LOH analizlerinin
KLL patogenezinin açıklanması için önemli olması sebebiyle array

yönteminin oldukça etkili olduğu savunulmuştur. Ayrıca delesyon 13q
büyüklüğünün ve saptanan diğer KSD’lerin prognostik etkisinin açığa
kavuşması için daha fazla çalışmaya ihtiyaç duyulduğu sonucuna
varılmıştır
Chronic lymphocytic leukemia (CLL) is the most common leukemia in
adults, and prognostic value of frequently recurrent chromosomal
abnormalities are discovered. Deletion of chromosome 13 [del(13q)] is the most
common chromosomal anomaly in CLL and it is considered as a good
prognostic biomarker when it is observed isolated. But it is also noticed that
clinical findings of patients with isolated del(13q) are quite heterogenous. In
our study, we aimed to investigate the relation between this clinical
heterogenity and size of del(13q) and other copy number variations (CNV) and
loss of heterozygosities (LOH) in CLL patients with del(13q).
In this study, 32 CLL patients with isolated del(13q) determined by
FISH method included and CNV & LOH analysis were performed by
aCGH+SNP method in DNA obtained by peripheral blood sample of 30
patients.
We found that 28/30 patients had del(13q). The size of deletions ranged
between 0,34 and 28,81 Mb, no correlation was found between the type of
deletions (type 1 / type 2 ) and clinical findings. Only one of our patients had
isolated del(13q), one had other CNV in a chromosomal region, 4 had in 2
chromosomal regions other patients had at least 3 CNVs except for del(13q).
The largest number of CNVs found in a patient was sixty one.
It is concluded that the size of del(13q) and the genes involved were very
heterogenous. The other CNVs found were rare CNVs, and one patient had
LOH involving ATM gene region. Out of the found CNVs; it is concluded that
gain of 16p13.3, NOTCH, COL1A1, 2p and loss of 18p11.32 may have a
prognostic value. Last of all, it is defended that array method is considerably
effective because analyses of CNVs, size of CNVs and LOHs are important to
explain the pathogenesis of CLL. Also, more studies are needed to figure out
the prognostic value of 13q deletion size and other detected CNVs

İntrakraniyal anevrizmalar (İA); beyindeki intrakraniyal arter duvarının bir defekt, hastalık veya hasara bağlı olarak geri dönüşümsüz anormal fokal dilatasyonudur. Birçok genetik ve çevresel risk faktörlerinden kaynaklanan sebepler nedeniyle toplumun %2-5‟ini etkileyen IA‟lar, travmatik olmayan subaraknoid kanamaların (SAK) yaklaşık %80-85‟inden sorumludur. Sigara, alkol kullanımı, hipertansiyon, diyabet gibi risk faktörlerinin yanı sıra tek yumurta ikizleri, ailesel genetik analizler ve popülasyon temelli genetik epidemiyoloji çalışmaları, genetik faktörlerin SAK ve İA patogenezinde oldukça önemli bir role sahip olduğunu göstermektedir. Günümüze kadar yapılan çalışmalar; ailesel İA olguları nedeniyle genetik yatkınlığın araştırılması yönündedir. Hastalık başlangıç yaşının ailesel İA‟ da daha erken olduğu, anevrizma rüptürüne bağlı SAK olasılıklarının ise toplumdan 7 kat daha fazla olduğu çalışmalarla ortaya konmuştur. Ailesel olarak görülmeleri İA‟ların ve rüptür gelişiminin genetik faktörlerden etkilenebileceğini göstermektedir. Yapılan çalışmalar İA‟nın multigenik olduğu yönündeki kanıtları destekler niteliktedir. Günümüzde İA‟nın tedavisinde en önemli zorluk anevrizmanın rüptürüdür. İntrakraniyal anevrizma rüptürü sonrası gelişen SAK morbidite ve mortalitesi oldukça yüksek bir durumdur. Bu nedenle anevrizmaların oluşumu ve rüptür riski ile ilişkili biyobelirteçlerin belirlenmesi moleküler patolojinin anlaşılması ve tedavi açısından büyük öneme sahiptir. Çalışmalarda rüptüre olan ve olmayan İA hastalarında kopya sayısı değişiklikleri (KSD) saptanmış ve KSD‟ ler ile SAK riski arasındaki ilişki araştırılmıştır. Ancak bu konuda veriler yetersiz kalmaktadır. Çalışmamızda; rüptüre olan ve rüptüre olmayan İA‟ lı hastalarda mikrodizin yöntemi ile KSD‟ lerin belirlenmesi, tespit edilen KSD‟ ler ile SAK arasındaki ilişkinin değerlendirilmesi amaçlanmıştır. Bu doğrultuda yirmi dört rüptüre ve yirmi dört unrüptüre olmak üzere toplamda kırk sekiz İA‟ lı hasta ve konfirmasyon amacıyla İA öyküsü bulunmayan, sağlıklı 8 kontrol olgusu çalışmamıza dahil edilmiş olup, hastaların kan örneklerinden izole edilen DNA örneklerinden mikrodizin yöntemi ile KSD‟ ler analiz edilmiştir. Çalışmamız sonucunda kanamış ve kanamamış olmak üzere tüm olgularımızda toplamda yüz otuz bir KSD saptanmıştır. Saptanan KSD‟ lerden birden fazla olguda iv ortak olarak saptananlar değerlendirmeye alınmıştır. Kanamış İA‟ lı olgularda birden fazla olguda saptanan KSD‟ ler; MSX1, SHH, GNAS, PPP2R3B ve TWIST1 genlerini içeren kromozomal lokusta kazanç ve GDI1 genini içeren kromozomal lokusta kaybı içermektedir. Kanamamış İA‟ lı olgularda birden fazla olguda saptanan KSD‟ ler; MSX1, SHH, GNAS genlerini içeren kromozomal lokusta kazanç, GDI1 ve NR2F2 genlerini içeren kromozomal lokusta kaybı içermektedir. Saptanan KSD bölgelerinin içerdikleri genler; DECIPHER, OMIM, DGV ve PubMed veritabanları kullanılarak incelenmiş ve İA ile doğrudan ilişkileri kurulamamış ancak literatür ile uyumlu lokuslar saptanmıştır. Ayrıca literatür verileri ile TWIST1 geni ve H19 polimorfik varyantının İA patogenezinde ilişkisi olabileceği saptanmıştır. İA patogenezinin anlaşılması için daha fazla veriye ve dolayısıyla olgu sayısı daha fazla olan yeni çalışmalara ihtiyaç duyulduğu sonucuna varılmıştır
Intracranial aneurysms (IA) are irreversible abnormal focal dilatation of the intracranial artery wall in the brain due to a defect, disease or damage. IAs that affect 2-5% of population due to reasons arising from many genetic and environmental risk factors, are responsible for approximately 80-85% of non-traumatic subarachnoid hemorrhages (SAH). In addition to risk factors such as smoking, alcohol use, hypertension, and diabetes, identical twins, familial genetic analyzes and population-based genetic epidemiology studies show that genetic factors play a very important role in the pathogenesis of SAH and IA. Studies conducted to date have been directed to the investigation of genetic predisposition due to familial IA cases. Studies have shown that the age of disease onset is earlier in familial IA, and the probability of SAH due to aneurysm rupture is 7 times higher than that of the population. Their familial appearance indicates that IAs and rupture development can be affected by genetic factors. Studies conducted support the evidence that IA is multigenic. Today, the most important risk in the treatment of IA is rupture of the aneurysm. SAH that develops after IA rupture is a situation with high morbidity and mortality. Therefore, determining the biomarkers associated with the formation of aneurysms and the risk of rupture is of great importance in terms of understanding molecular pathology and treatment. For these reasons, every study on this subject gains value. Clarification of genetic factors will lead to the determination of treatment strategies targeting pathophysiological processes, and risk assessments of presymptomatic individuals in the family can also be made. In the studies, copy number differences (CSD) were determined in IA patients with and without rupture and the relationship between CSDs and SAH risk was investigated. However, data on this subject are still insufficient.
In our study; it was aimed to determine the CNVs in patients with ruptured and unruptured IA by microarray method, and to evaluate the relationship between detected CNVs and SAH clinics. Accordingly, a total of 48 IA patients including 24 ruptured and 24 unruptured, and 8 healthy control individuals without a history of IA

for evaluation of detected CNVs were included into the study, and CNVs were analyzed by microarray method from DNA samples isolated from the blood samples of the patients and controls.
As a result of our study, a total of 131 CNVs was detected in all ruptured and unruptured IA cases. The CNVs detected in more than a case were taken into consideration. The gains in the MSX1, SHH, GNAS, PPP2R3B and TWIST1 genes were revealed in the patients with SAH whereas only the GDI1 gene deletion was seen in this group. CNVs detected in more than one case in patients with non-bleeding IA include gain in chromosomal loci containing MSX1, SHH and GNAS genes, and loss in loci containing GDI1 and NR2F2 genes.
The genes localized on the detected CNV regions were examined using the DECIPHER, OMIM, DGV and PubMed databases. Although the detected variants could not be directly related to IA, but loci were found in accordance with the literature. Literature data indicate that TWIST1 gene and H19 polymorphic variant may be related in IA pathogenesis, and our findings are consistent with the literature. It was concluded that more data and thus new studies with more cases are needed to understand the pathogenesis of IA

Konjenital Kalp Hastalığı (KKH); kalpte doğumda var olan yapısal veya fonksiyonel anomalilerdir ve toplumda en sık görülen konjenital malformasyonlar olup, yaklaşık %1-1.2 yenidoğanı etkiler. Her ne kadar KKH’ın altında yatan nedenler nispeten az anlaşılmış olsa da genetik ve çevresel faktörlerin etkisi olduğu düşünülmektedir. Epidemiyolojik çalışmalar genetik ve çevresel nedenlerin KKH vakalarının %20-30’unda gözlendiğini göstermiştir. Bu nedenle KKH'ın multifaktoriyel kalıtımlı olduğu kabul edilmektedir. Ayrıca ailesel izole KKH ile ilgili veriler, KKH'ın genetik temelinin de olduğunu ortaya koymaktadır. İzole/non-sendromik KKH tek gen veya kromozomal sendromlar dışında kalan, etiyopatogenezi henüz net olarak aydınlatılamamış, en yaygın KKH forrmudur. Bununla birlikte, izole/non-sendromik konjenital kalp hastalıklarının sadece % 20'si açıklanabilmektedir. Farklı DNA tamir mekanizmalarının neden olduğu submikroskopik değişiklikler; kopya sayısı varyasyonları (KSV) olarak tanımlanır ve konjenital anomaliler, sendromik/non-sendromik entelektüel yetersizlik ve otizm gibi birçok hastalığın oluşumunda rol oynar. Kromozom mikroarray analizi (KMA); tüm genomu yüksek rezolüsyonda inceleyerek dengesiz anomalilerin yanısıra varyantlarının da tespiti için kullanılan, genom çapında bir tarama tekniğidir. KSV’ler, izole/non-sendromik KKH etiyolojisinin %3-10’luk kısmından sorumludur. Non-sendromik KKH'li sporadik hastaların KMA ile incelenmesi, bu patolojilere katılan yeni genleri tanımlamak için klasik aile çalışmalarına bir alternatiftir. Sınırlı sayıda çalışmada, izole/non-sodromik KKH'li hastalarda bu yöntemi kullanarak ilişkili birkaç KSV tanımlanmıştır. Çalışmalar, submikroskopik KSV'lerin doğrudan veya genetik risk faktörleri olarak hastalık patogenezinde önemli bir rol oynadığına dair kanıt sağlamıştır. Türk popülasyonunda izole/non-sendromik KKH ile ilişkili KSV'lerin sıklığı ile ilgili veri bulunamamıştır.
İzole/non-sendromik KKH tanısı alan olgularda, KSV’lerin incelenmesi, frekanslarının belirlemesi ve olgularda saptanan bu KSV’lerin KKH tipleri arasındaki

dağılımları ile hastalık gelişimi üzerindeki etkilerinin değerlendirilmesi amacıyla toplam 64 (40 olgu, 24 kontrol) olgunun periferik kan örneklerinden elde edilen DNA örneklerinde aCGH+SNP yöntemi ile KSV’ler analiz edilmiştir.
Çalışmamız sonucunda toplam 33 olguda 62 KSV’nin 28 tanesi kayıp, 34 tanesi kazanç olarak saptanmıştır. Kontrol grubunda saptanan beş KSV'nin dört tanesi kayıp olup bir tanesi kazanç olarak saptanmıştır. Yapılan istatiksel analiz sonucunda KSV saptanan kontrol grubu ile araştırma grubu arasında anlamlı bir fark bulunmuştur (p=0.000243)
Congenital Heart Disease (CHD) is a structural or functional anomalies present at birth and it is the most common congenital malformations in the population, affecting about 1-1.2% newborns. Although the underlying causes of CHD remains relatively poorly understood, and it has been thought to have both genetic and environmental contributions. Epidemiological studies have shown that genetic or environmental causes are were in 20-30% of CHD cases. Therefore, it is accepted that CHD has a multifactorial inheritance. Isolated/ nonsyndromic CHD is the most prevalent form of CHDs in which etiopathogenesis has not been clearly elucidated except for single gene or chromosomal syndromes. Data on familial isolated CHD cluster reveal the genetic basis of CHD. However, only 20% of isolated/non-syndromic congenital heart diseases can be explained. Alterations of different DNA repair mechanisms causes submicroscopic changes which called copy number variants (CNV). CNVs plays a critical role during the various disease development which include congenital anomalies, syndromic/nonsyndromic intellectual insufficiency and autism. Chromosome microarray analysis (CMA) is a genome-wide screening technique that is used for the detection of unbalanced anomalies as well as copy number variants by examining the whole genome at high resolution. CNVs are responsible for 3-10% of the etiology of isolated/nonsyndromic CHDs. The study of sporadic patients with nonsyndromic CHD with CMA is an alternative to classic family studies to identify new genes involved in these pathologies. A limited number of studies have identified several associated CNVs using this method in patients with isolated/nonsyndromic CHD. Studies provided evidence that submicroscopic CNVs play an important role for disease pathogenesis, either directly or as genetic risk factors. No data could be found about the frequencies of isolated/nonsyndromic CHD related CNVs in Turkish population.

among CHD types and their effects on disease development, DNA samples obtained from the peripheral blood samples of 64 (40 cases, 24 controls) CNVs were analyzed by the aCGH + SNP method.
As a result of our study, 28 of 62 CNVs in a total of 33 cases were found to be lost and 34 were found to be gain. 4 of 5 CNV detected in the control group were loss and 1 was found as gain. As a result of the statistical analysis, a significant difference was found between the control group with CSV and the study group. (p = 0.000243).

KRONİK MYELOİD LÖSEMİ OLGULARINDA İLAVE KLONAL KROMOZOM ANOMALİLERİNİN RETROSPEKTİF İNCELENMESİ
Kronik myeloid lösemi (KML) kan hücelerini oluşturan projenitör kök hücrelerde tutulum gösteren bir kanser türüdür. Hastalarda 9 ile 22 numaralı kromozomların resiprokal translokasyonu ile karakterizedir. KML hastaların %15-%20’sinde Philadelphia kromozomuna ek olarak gözlenen ek sitogenetik anomaliler (ACA) bulunur. Bu ACA’lar prognostik etkilerine göre major ve minor ACA’lar olmak üzere iki gruba ayrılır.
Biz çalışmamızda 2009-2019 yılları arasında Eskişehir Osmangazi Üniversitesi (ESOGÜ) Tıp Fakültesi Tıbbi Genetik Anabilim Dalı’nda KML tanılı olguların kemik iliği örneklerinden çalışılan konvansiyonel sitogenetik ve FISH testleri sonucunda saptanan varyant t(9;22) ve Ph kromozomuna ek kromozomal anomalilerin tespit edilme sıklığını belirlemeyi amaçladık. Ayrıca çalışmamız sırasında klinik bilgilerine ulaşılan hastaların yanıt oranlarını da çalışmamıza ekledik ve bu oranlardan yola çıkarak gözlenen bu ek anomalilerin tedaviye olan etkilerini de değerlendirdik.
Bölümümüze KML tanısı alarak yönlendirilen dört yüz elli iki hasta içerisinden varyant ph kromozomu veya ph kromozomuna ek sitogenetik anomalisi bulunan otuz altı (%8) hasta tespit ettik. Bu hastalar içerisinde en sık gözlenen anomalinin 6 olgu ile (% 17) ile ilave Philadelphia kromozomu olduğunu gördük. Ayrıca olgularımızın 4 tanesinde (%11) trizomi 8, 3 tanesinde (%8) Y kromozomu kaybı, 3’ün de (%8) varyant Philadelphia translokasyonu, 1 hastamızda trizomi 9, 1 hastamızda 9q34 delesyonu ve 1 hastamızda ise 6 numaralı kromozomun trizomisini gözledik.
Çalışmamızın sonunda KML olgularında ACA saptama oranımız literatür verisinin altındadır. Ayrıca literatür verilerine göre sınıflandırdığımızda 10 olgunun (%28) major yolak anomalilerine, 26 olgunun da (%72) minor yolak anomalilerine sahip olduğunu gördük. Major ve minor yolakta ki bu dağılım verisinin literatürde yer alan oranlar ile uyumlu olduğunu saptadık.
Tespit edilen anomalilerin tedaviye yanıt oranları incelediğinde varyant t(9;22)’ler haricinde major ve minör yolağa ait anomali gruplarındaki verilerin literatür ile uyumlu olduğunu gözledik. Ancak varyant translokasyona yönelik beklenen kötü yanıtı bizim olgu grubumuzda görmedik. Bununla birlikte hem çalışmamızda hem de literatürde olgu sayımızın az olması sebebiyle saptanan anomalilerin tedaviye olan etkileri konusunda kesin sonuçlar elde edemedik. Ayrıca

çalışmamızda hastaların bir kısmında elde edilen metafaz plaklarının sitogenetik değerlendirme için yetersiz olduğunu gördük. Bu nedenle tüm KML vakalarımız arasında ortaya çıkarılamayan ACA ve varyant t(9,22)’lere sahip olgular olduğunu düşünmekteyiz. Bu da bize konvansiyonel sitogenetik analiz yapılmasının kanser genetiğinde çok önemli olduğunu göstermiştir
RETROSPECTIVE INVESTIGATION OF ADDITIONAL CLONAL CHROMOSOMES ANOMALIES IN CHRONIC MYELOID LEUKEMIA
Chronic myeloid leukemia (CML) is a type that shows involvement in progenitor stem cells in blood cells. It is characterized by reciprocal translocation of chromosomes 9 to 22 in patients. 15% -20% of chronic myeloid leukemia patients have additional chromosomal anomalies (ACA) observed in addition to the Philadelphia chromosome. In our study, we detected variant t (9; 22) and additional chromosomal abnormalities in the Ph chromosome, which were detected as a result of conventional cytogenetic and FISH tests, which were studied from bone marrow samples of patients with CML at Eskişehir Osmangazi University (ESOGÜ) Medical Genetics Department between 2009-2019. We aimed to determine the frequency of occurrence. In addition, we included the treatment response rates of patients whose information was obtained during our study, and based on these rates, we aimed to evaluate the effects of these additional anomalies on treatment.
Among 452 patients who were referred to our department with a diagnosis of CML, we established a case series of 36 patients, in which we found that variant ph chromosome or additional cytogenetic anomaly to the ph chromosome. In this series, with 6 cases (17%), the additional Philadelphia chromosome is the most common additional anomaly. In addition to that, trisomy 8 in 4 of our cases (11%), Y chromosome loss in 3 cases (8%), variant Philadelphia translocation in 3 cases (8%), trisomy 9 in 1 patient, 9q34 deletion in 1 patient, and trisomy of chromosome number 6 in 1 patient were also observed.
At the end of our study, our ACA detection rate in CML cases is below the literature data. In addition, when we classified according to the literature data, we found that 10 cases (28%) had major pathway anomalies and 26 cases (72%) had minor pathway anomalies. We found that this distribution data in the major and minor pathway is compatible with the rates in the literature.
When we examined the response rates of the detected anomalies to treatment, we observed that the data in the anomaly groups belonging to the major and minor pathways, except for the variant t (9; 22), were compatible with the literature. However, we did not see the expected poor response to variant translocation in our case group. However, due to the small number of cases in our study and in the literature, we could not obtain definitive results about the effects of the detected anomalies on treatment. In addition, in our study, we found that the metaphase plates obtained in some of the patients were insufficient for cytogenetic evaluation. For this reason, we think that there are cases with ACA and

variant t (9.22) that cannot be detected among all our CML cases. This has shown us that conventional cytogenetic analyzes are very important for cancer genetics
